Visceral leishmaniasis is an important neglected parasitic disease that is generally caused by Leishmania infantum, Leishmania donovani and Leishmania chagasi. However, several causative species of cutaneous leishmaniasis (CL) causes an interstitial form of leishmaniasis which known viscerotropic leishmaniasis. The aim of this paper is a systematic review of the cases of viscerotropic leishmaniasis to present the main causative agents, clinical manifestations, treatment and outcomes of the cases. An electronic search (any date to August 2017) without language restrictions was performed using Medline, PubMed, Scopus and Google Scholar. The searches identified 19 articles with total 30 case reports. Of them, old world Leishmania species was reported from 23 (76.7%) cases, including 20 cases of L. tropica and three cases of L. major; whereas new world Leishmania species were reported in seven (23.4%) cases. The infection was more prevalent in male (24/30, 80%) than female (5/30, 16.7%) patients. Co-morbidity/co-infection was observed in 13 out of 30 cases (43.4%), which the most of them was HIV/AIDS (10 out of 13 cases, 76.9%). The results suggested that viscerotropic leishmaniasis should be more attended in the endemic countries of CL and in immunocompromised patients in order to exact discrimination from other endemic infectious diseases.

Objectives: The health technology assessment (HTA) Core Model® is a tool for defining and standardizing the elements of HTA analyses within several domains for producing structured reports. This study explored the parallels between the Core Model and a national HTA report. Experiences from various European HTA agencies were also investigated to determine the Core Model's adaptability to national reports.

Methods: A comparison between a national report on Genetic Counseling, produced by the Cellule d'expertise médicale Luxembourg, and the Core Model was performed to identify parallels in terms of relevant and comparable assessment elements (AEs). Semi-structured interviews with five representatives from European HTA agencies were performed to assess their user experiences with the Core Model.

Results: The comparative study revealed that 50 percent of the total number (n = 144) of AEs in the Core Model were relevant for the national report. Of these 144 AEs from the Core Model, 34 (24 percent) were covered in the national report. Some AEs were covered only partly. The interviewees emphasized flexibility in using the Core Model and stated that the most important aspects to be evaluated include characteristics of the disease and technology, clinical effectiveness, economic aspects, and safety.

Conclusions: In the present study, the national report covered an acceptable number of AEs of the Core Model. These results need to be interpreted with caution because only one comparison was performed. The Core Model can be used in a flexible manner, applying only those elements that are relevant from the perspective of the technology assessment and specific country context.

Objectives: The HTA Core Model is a framework for producing health technology assessments (HTAs) in a structured format. The Model splits the content of a HTA into assessment elements. The objective is to explore the adaptability of these assessment elements in national report production in a pilot case study comparing a national HTA report and the HTA Core Model.

Methods: An on-going Dutch HTA report on endovascular repair of abdominal aortic aneurysm (EVAR) was chosen as a typical representative of a national report on medical interventions. The author of the EVAR report assessed the relevance and comprehensiveness of the assessment elements of the HTA Core Model for her work. Another researcher annotated the Core Model specific content in the EVAR report. Matching and missing content, as well as the distribution of information in the EVAR report were tabulated and analysed in joint deliberations.

Results: Forty percent of the assessment elements of the Core Model were considered relevant for the EVAR report. Some issues relevant for EVAR but missing from the Core Model were identified: they were about re-interventions, secondary prevention, subpopulations that benefit most, and the length of the hospital stay. The distribution of information differed substantially between the Code Model and the national report.

Conclusions: The assessment elements of the HTA Core Model covered most relevant questions of the national report. In order to facilitate easy adaptation of information, the distribution of information should be more consistent in the national report and the Core model.

Two cases of intestinal obstruction due to Ascaris lumbricoides infection are reported from Ishaka Adventist Hospital, in south western Uganda. One was a male aged two and a half years and another was a female aged twenty. They were both treated surgically with good results. A review of the literature also revealed that intestinal obstruction due to A. lumbricoides is a common complication, though other surgical and nutritional complications also seem to be common, they are less reported.

The extent to which letter-by-letter reading results from a specific orthographic deficit, as compared with a nonspecific disturbance in basic visuoperceptual mechanisms, is unclear. The current study directly compared processing of letters and digits in a letter-by-letter reader, G.M., using a rapid serial visual presentation (RSVP) task and a speeded matching task. Comparisons were made to a group of six brain-damaged individuals without reading deficits. In the RSVP task, G.M. had increased difficulty reporting the target identities when they were letters, as compared with digits. Although this general pattern was also evident in the control group, the magnitude of the letter–digit accuracy difference was greater in G.M. Similarly, in the matching task, G.M. was slower to match letters than digits, relative to the control group, although his response times to both item types were increased. These data suggest that letter-by-letter reading, at least in this case, results from a visuoperceptual encoding deficit that particularly affects letters, but also extends to processing of digits to a lesser extent. Results are consistent with the notion that a left occipitotemporal area is specialized for letter processing with greater bilaterality in the visual processing of digits. (JINS, 2008, 14, 164–173.)

Theories of the cognitive impairment underlying letter-by-letter reading vary widely, including prelexical and lexical level deficits. One prominent prelexical account proposes that the disorder results from difficulty in processing multiple letters simultaneously. We investigated whether this deficit extends to letters presented in rapid temporal succession. A letter-by-letter reader, G.M., was administered a rapid serial visual presentation task that has been used widely to study the temporal processing characteristics of the normal visual system. Comparisons were made to a control group of 6 brain-damaged individuals without reading deficits. Two target letters were embedded at varying temporal positions in a stream of rapidly presented single digits. After each stream, the identities of the two letters were reported. G.M. required an extended period of time after he had processed one letter before he was able to reliably identify a second letter, relative to the controls. In addition, G.M.'s report of the second letter was most impaired when it immediately followed the first letter, a pattern not seen in the controls, indicating that G.M. had difficulty processing the two items together. These data suggest that a letter-by-letter reading strategy may be adopted to help compensate for a deficit in the temporal processing of letters (JINS, 2007, 13, 110–119.)
