This paper is a narrative review examining the high prevalence of care leaver early parenting in the context of (i) key transitions from care studies taken from the last few decades, (ii) a structured review using Scopus of studies from 2015–2020 focussed specifically on young people transitioning from care and early parenting and (iii) Boss’s (2010) Ambiguous Loss theory. Young care leavers’ challenges, in general, put them at higher risk of protective interventions with their children and may contribute to the growing numbers of children being placed in increasingly over-stretched out-of-home care systems. Questions of surveillance bias for service-connected young people are examined in light of recent large-scale studies using administrative data sets. Serious oversights in responding to young people’s experiences of trauma and exploitation are identified. The relevance of sexual health programs for young people actively seeking pregnancies is discussed with emerging evidence that disengagement from schooling may have more of a role in explaining early pregnancy and parenting than previously thought. The authors test the relevance of Ambiguous Loss theory in understanding how removal from families of origin and placement experiences may affect young people and lead to ‘wanted’ pregnancies.

Internationally, the care-leaving debate began in the 1970s. The poor outcomes associated with care-leaving in the United Kingdom, United States of America and Australia prompted attention resulting in policy change in recent years, which continues to develop. The experience and outcomes for care leavers in South Africa reflects that of their contemporaries in other countries, however, contextual factors compound the problems that they face and there is little support available to them. This paper discusses some of the challenges facing care leavers and the development of the care-leaving debate, legislation and policy in the United Kingdom, United States and Australia. A comparison of the care-leaving arena in South Africa and the support services available to care leavers in the different countries will be presented. The paper concludes by arguing that the absence of services for care leavers is a neglect of the state's responsibility as corporate parent, and represents an issue of social justice.

This research explored the experiences of care leavers, who lived in institutions (such as Children's Homes and orphanages) or other forms of out-of-home care between 1930 and 1989. Participants included representatives of three sub-cohorts: Forgotten Australians, members of the Stolen Generations and Child Migrants. Employing mixed methods, this research used three forms of data collection: surveys (n = 669), interviews (n = 92) and focus groups (n = 77). This research concentrated on participants’ experiences in care, leaving care, life outcomes after care (education, employment, health, wellbeing and relationships), coping strategies and resilience, current service needs and usage and participation in organisations as well as the Royal Commission into Institutional Responses to Child Sexual Abuse. Most participants experienced extreme neglect and abuse while in care. Leaving care, often after years of institutionalisation, was generally a frightening and demoralising process. Despite these challenges, a number of participants demonstrated remarkable resilience. For many, however, these experiences had negative consequences in adulthood including serious physical and mental health problems. This often made adult learning, paid employment and positive relationships virtually impossible. Most survivors carry high levels of trauma and complex unmet needs. Implications for policy, practice and services are drawn from key findings.

Young people transitioning from out-of-home care (OHC) are a vulnerable group. One particular manifestation of disadvantage is poor mental health outcomes which may reflect both the traumatic effects of childhood abuse, and a lack of support on leaving care. This article presents the findings of a small qualitative study undertaken in Victoria which explored the views of OHC and mental health service providers regarding the mental health support needs of care leavers. The findings are consistent with existing research results internationally in highlighting a number of key factors that influence mental health outcomes including the impact of pre-care, in-care and transition from care experiences.

Young people transitioning from out-of-home care are known to have poor educational outcomes compared to their non-care peers. Yet little is known about the experiences or needs of the small numbers of Australian care leavers who enter higher education. This article critically examines existing Australian and international research on the access of care leavers to higher education. A group of pre-care, in-care, transition from care and post-care factors are identified as either hindering or assisting care leavers to maximise their educational opportunities. Some specific policy and practice reforms are recommended to enhance opportunities for Australian care leavers to participate in and complete higher education.

This article reviews initial findings from an Australian Housing and Urban Research Institute [AHURI]-funded research project examining housing outcomes among Australian young people who have been in state out-of-home care. Our findings suggest a linkage between incidents of in-care abuse and poor postcare housing outcomes among our research participants, including primary homelessness. Not attending school when leaving care was also highly associated with having experienced in-care abuse. The authors postulate that adverse in-care experiences may have contributed to poor postcare housing outcomes among the research participants; and this article raises a number of specific concerns related to neglect, abuse and assault while in care. It is also argued that support in the transition from care needs to be strengthened to mitigate poor post-care outcomes, as does accountability for in-care adversities.
