This chapter highlights some important features of psychological work within the context of acute psychiatric services, specifically psychiatric intensive care unit (PICU) settings. A psychological perspective offers a valuable opportunity for mental health professions to investigate the holistic experience of ward culture and patient treatment on such wards. This chapter explores some of the key roles of a clinical psychologist on a PICU, including offering psychological assessments, psychological therapy and management to patients and their families. A relational and developmental model is used, in line with object-relations theorists such as Winnicott and Casement. The chapter reflects on the role of psychologists in offering staff support and encouraging psychological safety on wards. It explores specific challenges to psychological work on acute wards will be explored, including working with the involuntary status of patients, and the complexity and risk to self and others inherent to presentations in acute settings.

Responders in the UK attend a range of emergencies, incidents, disasters, and disease outbreaks (EIDD). Although the impacts on professional responders have been researched, documented, and are reasonably well understood, the consequences of that exposure to such incidents for their families is relatively underexplored. This chapter identifies the links between the wellbeing of families of responders and the impacts from the occupation of their emergency service family members. The support given by family members is explored in the context of social support, diffusing, and managing mental health. This is informed by empirically evidenced research projects detailing the occupational consequences for the families of responders through literature and national project work.

Chapter 2 draws heavily on surviving letters between inmates and their loved ones, listed addresses in prisoners’ files and visitor records in order to explore the impact of separation and ways families attempted to sustain bonds across lengthy prison sentences. Through four distinct sections, it offers a fascinating insight into family relationships, domestic arrangements, expected responsibilities and obligations. The first section examines the ways imprisoned women sought to maintain contact with their loved ones and vice versa through letter writing. The second section focuses on visitors received by inmates, revealing desires and obligations within the family unit. The third section examines convict mothers’ relationships with their children, some of whom were born in prison and others of whom accompanied their mothers to penal servitude. Changes to legislation and practices across the century restricted convict mothers’ time with their offspring and, towards the end of the century, meant that women with children had to find alternative means to fulfil mothering roles. The final section considers the influence relatives and friends could have on a convict’s release. While it is apparent that family relationships could be maintained, this chapter also shows evidence of strained relationships between incarcerated women and their families and friends.

Unaccompanied young refugees form a particular group amongst refugee children and adolescents. Although they are often identified as a homogenous group, they largely differ, such as in their educational backgrounds, migration motives and expectations, mental health status and wellbeing, and familial contexts. The process of familial separation throughout the migration trajectory and settlement process leads to complex and dynamic processes of searching for renewed equilibriums between new social networks in the host country, maintaining relationships with and receiving support from family members in the home country and meeting (supposed) expectations from and continuous caring for family members back home. Given the important of social support and family loyalties to the wellbeing of these young refugees, both practitioners and policy-makers need to take hold of these complex aspects in their daily care practices and policy-making.
