  • A novel prognostic biomarker for recurrent aphthous stomatitis: calprotectin

  • Objective

    This study aims to investigate whether there is a correlation between serum calprotectin concentration and oral ulcer activity in patients with active recurrent aphthous stomatitis.

    Method

    This prospective, cross-sectional, historical cohort study included 52 patients (26 with active lesions and 26 with inactive lesions) with recurrent aphthous stomatitis and 27 healthy volunteers. The relationship between the groups was statistically evaluated in terms of serum calprotectin level and other inflammatory parameters.

    Results

    The median calprotectin value of the active lesion group (80.5 ng/ml) was significantly higher than that of the control group (55.4 ng/ml, p = 0.002). Furthermore, the median calprotectin value of the active lesion group was significantly higher than that of the inactive lesion group (72.5 ng/ml, p = 0.017).

    Conclusion

    In patients with recurrent aphthous stomatitis and active ulcer lesions, the serum calprotectin value can be used as a reliable and robust prognostic marker for inflammation.

  • Objective:

    Oral mucositis (OM) is a frequently encountered problem as a complication of cancer treatment. We investigated whether daily washings with colchicine solution improved mucositis in patients with hematological malignancies undergoing chemotherapy.

    Methods:

    This study was a one-arm, nonrandomized clinical trial that used a historical control group. Patients were included in the study from the first day of mucositis and followed up until discharge. Patients received 2 mg colchicine mouthwashes daily for 5 days or saline solution. OM was assessed once daily until symptom resolution, using the WHO grading scale of 0–4 and a visual analogue scale. We determined that at least 40 patients in the colchicine group would be needed to detect a 20% difference in the duration of OM between Groups A and B, with a 95% confidence level and a power of 80%.

    Results:

    82 patients were included in the final analysis, 40 in the colchicine group and 42 in the control group. Median duration of OM was significantly different among groups; 9days (range 1–17days) for the control group versus 6days (range 3–13days) for those exposed to colchicine mouthwash (p = .028). The median days of regression of mucosal lesions were significantly different (p = .047) among the control group (7 days [range 3–20]) compared to the colchicine group (4 days [range 2–14]).

    Significance of results:

    Although our findings suggest that colchicine mouthwash is helpful in reducing the severity and duration of chemotherapy-induced OM, randomized trials are needed to confirm these results.

  • Summary

    Available evidence for treating pain caused by mucositis (inflammation of the digestive tract's lining) and stomatitis (mucositis involving the mouth) can be categorized into two general categories, depending on whether or not the stomatitis is related to cancer therapy (either chemo-or radiation therapy). This chapter addresses cancer treatment-associated stomatitis (CTAM). For CTAM that does develop, pain can be severe and opioids are the therapeutic mainstay. In fact, CTAM pain often necessitates IV morphine and even hospitalization and institution of a PCA regimen. Local anesthetics such as tetracaine or lidocaine are often recommended for CTAM, but their utility is not definitively demonstrated. Most of the data dealing with non-CTAM mucositis addresses aphthous ulcer (AU) treatment. Topical sucralfate is an example of an approach that is not successful in CTAM but found useful in AU. The chapter concludes with an overview of the evidence, lesser in quality and quantity, addressing non-CTAM stomatitis.

  • We present a case of oral ulceration in a patient with essential thrombocythaemia, that proved to be secondary to treatment with hydroxyurea. This complication has not yet been reported in the ENT literature. Hydroxyurea should be considered as a possible cause of persistent oral ulceration in patients with haematological conditions. It should be noted that the onset of this complication is not related to the dose and duration of therapy.
