Colloid fluids are crystalloid electrolyte solutions with a macromolecule added that binds water by its colloid osmotic pressure. As macromolecules escape the plasma only with difficulty, the resulting plasma volume expansion is strong and lasts many hours. The clinically used colloid fluids include albumin, hydroxyethyl starch, gelatin, and dextran.
The plasma volume expansion shows one-compartment kinetics. Marketed iso-oncotic fluids are usually composed so that the infused volume expands the plasma volume by the infused amount. Exceptions include hyperoncotic variants such as 20% albumin.
The main indication for colloid fluid is as second-line treatment of hemorrhage. Because of inherent allergic properties, crystalloid electrolyte fluids should be used when the hemorrhage is small. A changeover to a colloid should be performed only when the crystalloid volume is so large that adverse effects may ensue. The only other clinical indication is that dextran can be prescribed to improve microcirculatory flow.
Chronic rhinosinusitis patients with biofilms cultured from their sinonasal cavity have greater symptom burden and risk of recalcitrant disease. A number of non-antibiotic, ‘anti-biofilm’ treatments exist which show anti-biofilm properties in preclinical studies. There is little evidence evaluating their impact on clinical symptom scores in chronic rhinosinusitis.
Method
A systematic review was performed to assess the literature regarding the efficacy of non-steroid, non-antibiotic, anti-biofilm specific topical therapies in the treatment of chronic rhinosinusitis. The primary outcome assessed was change in validated patient reported outcome measures before and after anti-biofilm treatment.
Results
Thirteen studies assessing the effect of anti-biofilm therapies in chronic rhinosinusitis through validated patient-reported outcome measures were included. Seven different anti-biofilm specific therapies for chronic rhinosinusitis were identified. None of the seven anti-biofilm therapies was identified as being confidently efficacious beyond placebo. Only one therapy (intranasal xylitol) showed a statistically significant reduction in symptom scores compared with placebo in more than one trial.
Conclusion
Robust evidence supporting the use of various anti-biofilm therapies in chronic rhinosinusitis is lacking. Further high quality, human, in vivo trials studying the effect of anti-biofilm therapies in chronic rhinosinusitis are needed to address the deficiencies of the current evidence base.
The Temperament and Character Inventory (TCI) is commonly used in adult populations. Our aim was to explore: (1) if there are specific differences in temperament dimensions related to depression in comparison with general population, (2) if the treatment response during the acute phase of major depressive disorder (MDD) is predictable by TCI temperament dimensions.
Method
Temperament profiles in 98 MDD patients were compared with a Finnish community sample. The patients were treated with serotonin selective reuptake inhibitors (SSRIs) for 6weeks and their temperament profiles were assessed at baseline and endpoint. The harm avoidance (HA) and depression scores at baseline and endpoint were modelled with path analysis. For path modelling, we tested the relationships between different temperament dimensions and depression symptoms and other clinical variables with Mancova model.
Results
The HA scores were significantly higher in patients both at baseline and endpoint compared to the Northern Finland 1966 Birth Cohort (NFBC). The patients, and especially males, had slightly higher reward dependency (RD) scores. HA at endpoint explained moderately the Montgomery Åsberg Depression Rating Scale (MADRS) endpoint score. HA endpoint score was strongly explained by HA baseline score.
Conclusions
HA is associated with risk of and treatment response to depression.
