Convocatio sive concilium provinciale reverendissimi in Christo patris et domini Domini Richardi, providentia divina Eboracensis archiepiscopi, Angliae primatis et metropolitan, ac ceterorum praelatorum et cleri civitatis, dioeceseos et provinciae Eboracensis, auctoritate brevis excellentissimi in Christo principis et domini nostri Domini Caroli, Dei gratia Angliae, Scotiae, Franciae et Hiberniae regis, fidei defensoris etc., convocatorum inchoata sive inchoatum decimo quarto die mensis Aprilis anno Domini millesimo sexcentesimo quadragesimo, regnique dicti domini nostri regis anno decimo sexto, in domo capitulari ecclesiae cathedralis et metropoliticae Beati Petri Eboracensis, inter horas nonam et undecimam ante meridiem eiusdem diei, coram praefato reverendissimo in Christo patre praeside convocationis eiusdem sive consilii provincialis, in praesentia nostrum Gulielmi Turbatt, actorum notarii publici et Gulielmi Freer et Thomae Thompson notariorum etiam publicorum.

Quibus die, horis et loco, publice perlecto de mandato reverendissimi patris per me notarium publicum antedictum brevi regio antedicto reverendissimo patri directo in haec verba:

Carolus, Dei gratia Angliae, Scotiae, Franciae et Hiberniae rex, fidei defensor etc., reverendissimo in Christo patri ac fideli consiliario nostro Richardo, archiepiscopo Eboracensi, Angliae primati et metropolitano, salutem. Quibusdam arduis et urgentibus negotiis nos, securitatem et defensionem Ecclesiae Anglicanae ac pacem et tranquillitatem, bonum publicum et defensionem regni nostri et subditorum nostrorum eiusdem concernentibus, vobis in fide et dilectione quibus nobis tenemini rogando mandamus, quatenus praemissis debito intuitu attentis et ponderatis, universos et singulos episcopos vestrae provinciae ac decanos ecclesiae cathedralis [6] necnon archidiaconos, capitula et collegia totumque clerum cuiuslibet dioeceseos eiusdem provinciae ad comparendum coram vobis in ecclesia metropolitica Sancti Petri Eboracensis decimo quarto die mensis Aprilis proxime futuri, vel alibi prout melius expedire videatis, cum omni celeritate accommoda modo debito convocari faciatis, ad tractandum, consentiendum et concludendum super praemissis et aliis quae sibi clarius exponentur tune ibidem ex parte nostra. Et hoc sicut nos et statum regni nostri et honorem et utilitatem Ecclesiae praedictae diligitis, nullatenus omittatis.
