Health anxiety (HA) is common in psychiatric and medical settings. Cognitive models of HA highlight the role of misinterpreting physical sensations as dangerous. This report presents the case of a 31-year-old man and the use of a cognitive-behavioural approach to treat his HA which also considers the role of misinterpreting intrusions as abnormal, by drawing on theoretical accounts of obsessive-compulsive disorder (OCD). A single-case experimental design demonstrated reliable improvements in symptom measures of HA and general distress. Distinguishing sensation-based versus intrusion-based appraisals in HA has implications for interventions in health settings and for refining cognitive theory.

Rumination has been shown to play a part in post-traumatic stress disorder (PTSD), but its relation to the intrusions characteristic of PTSD has mainly been investigated experimentally. This proof-of-concept case study explored the occurrence, personal experiences, and possible relation between rumination and intrusions in two PTSD patients in their daily living using a mixed method approach. A novel wearable self-tracking instrument was employed which provided fine-grained temporal resolution of observation data and could eliminate recall bias. Furthermore, quantitative and qualitative data were collected on participants’ symptoms, rumination and experiences of using the self-tracking instrument. First, without distinguishing between the two phenomena, the participants tracked both for a week. After receiving psychoeducational training for distinguishing between rumination and intrusions, the differentiated phenomena were tracked for a week. Both participants reported being subjectively able to distinguish between rumination and intrusions and made observations with high adherence during the project. Data hinted at a possible temporal relation between the phenomena in line with theories posing rumination as a maladaptive coping strategy as well as an exacerbator of PTSD symptoms. However, relations to mood were inconclusive. Furthermore, by using the self-tracking instrument, participants gained a heightened awareness of the characteristics of rumination and intrusions and contextual cues for occurrence, as well as a greater sense of momentary agency. Results reveal promising prospects in using the wearable self-tracking instrument for further investigation of the relation between rumination and intrusions in the lived lives of PTSD patients, as well as potential for incorporating this method in clinical treatment.

Trauma-focused cognitive behavioural therapy (TF-CBT) is beneficial for individuals with post-traumatic stress disorder (PTSD); however, a subset of clients struggle to engage with traditional methods, due to high levels of avoidance and dissociation. This paper aims to describe an adapted approach to imaginal reliving and prolonged exposure, to facilitate subsequent cognitive updating. The paper demonstrates the technique with veterans, who are a client group that may struggle with some aspects of traditionally implemented TF-CBT. Two case studies are described, both with PTSD symptoms stemming from traumatic military experiences. An adapted exposure technique is utilized to address the barriers of high dissociation, poor access to trauma-related cognitions and fixed intrusive imagery. The approach involved three stages: (1) reliving the trauma outdoors, (2) manipulating the perspectives of the imagery, and (3) restructuring the narrative with new perspectives. Both clients showed decreased dissociation and improved toleration of their traumatic imagery. Improvement of PTSD symptoms and quality-of-life functioning was observed for both clients on objective measures. Adapting TF-CBT to have a stronger emphasis on grounding and allocentric processing may be helpful for a subset of patients with PTSD that present with high levels of dissociation and avoidance. Further research and investigation into alternative populations is needed.

Radiometric ages for undated parts of the volcanic succession and intrusions in West Greenland were obtained by the 40Ar–39Ar incremental heating method. Acceptable crystallization ages were obtained for 27 samples. Combined with published results the new data provide a volcanic stratigraphy correlatable throughout the Nuussuaq Basin. The thick onshore volcanic pile consists of four widespread formations: 62.5–61 Ma picrites (Vaigat Formation), 61–60 Ma depleted basalts (Maligât Formation and the Hellefisk-1 well), 60–58 Ma less-depleted basalts (Svartenhuk Formation) and 56–54 Ma enriched basalts (Naqerloq Formation). Two local successions comprise 53.5 Ma alkali basalts (Erqua Formation) and 38.7 Ma transitional basalts (Talerua Member). A central volcano developed on Ubekendt Ejland, leading to the Sarqâta qáqâ gabbro-granophyre intrusion at 57–55 Ma. Pre-break-up volcanism took place further south as early as 64–63 Ma. The offshore volcanic succession most probably comprises the known onshore succession plus some younger lavas. The change in spreading direction near the Paleocene–Eocene boundary took place west of the Nuussuaq Basin c. 56.2 Ma. Some tectonomagmatic events are correlatable across the entire North Atlantic Igneous Province. A quiescent 58–56 Ma period correlates with similar periods in East Greenland and the Faroes, and the Naqerloq Formation is coeval with the Eocene basalts in East Greenland. The Paleocene and Eocene tholeiitic basalts are distinguishable chemically; in early Eocene time, mantle typical of the Iceland plume seems to have extended beneath the whole West Greenland margin as well as the central East Greenland margin.

Bias in self-reported dietary intake is important when evaluating the effect of dietary interventions, particularly for intervention foods. However, few have investigated this in children, and none have investigated the reporting accuracy of fish intake in children using biomarkers. In a Danish school meal study, 8- to 11-year-old children (n 834) were served the New Nordic Diet (NND) for lunch. The present study examined the accuracy of self-reported intake of signature foods (berries, cabbage, root vegetables, legumes, herbs, potatoes, wild plants, mushrooms, nuts and fish) characterising the NND. Children, assisted by parents, self-reported their diet in a Web-based Dietary Assessment Software for Children during the intervention and control (packed lunch) periods. The reported fish intake by children was compared with their ranking according to fasting whole-blood EPA and DHA concentration and weight percentage using the Spearman correlations and cross-classification. Direct observation of school lunch intake (n 193) was used to score the accuracy of food-reporting as matches, intrusions, omissions and faults. The reporting of all lunch foods had higher percentage of matches compared with the reporting of signature foods in both periods, and the accuracy was higher during the control period compared with the intervention period. Both Spearman's rank correlations and linear mixed models demonstrated positive associations between EPA+DHA and reported fish intake. The direct observations showed that both reported and real intake of signature foods did increase during the intervention period. In conclusion, the self-reported data represented a true increase in the intake of signature foods and can be used to examine dietary intervention effects.

Background: Improving mental health literacy in the general population is important as it is associated with early detection and treatment-seeking for mental health problems. Target areas for mental health literacy programs should be guided by research that tests the impact of improving knowledge of psychological constructs associated with the development of mental health problems. Aims: This study investigated the impact of providing corrective information about the nature of intrusive thoughts on their subsequent appraisal in a community sample. Method: In an online, experimental design, 148 community participants completed measures of obsessive-compulsive symptoms and appraisals (Obsessive Compulsive Inventory-Revised [OCI-R]; Intrusions Inventory [III]). Individuals were instructed to read either a brief informational text about the nature of intrusive thoughts or a control text. All participants then completed post-test measurements of appraisals. Intervention effectiveness was analysed using hierarchical multiple regression. Results: Individuals in the intervention group reported significantly lower levels of maladaptive appraisals than those in the control group (α = .05). Conclusions: The results of this study support the efficacy of provision of brief written information in reducing negative appraisals of intrusive thoughts in a community sample. It suggests a possible role for education about intrusive thoughts as a prevention strategy for obsessive-compulsive disorder.

Introduction: Research has indicated that individuals may report benefit finding (BF) as a result of experiencing potentially life-threatening traumatic events, such as being diagnosed with cancer or sustaining an acquired brain injury (ABI). No study to date has examined the incidence and correlates of BF in adult brain tumour samples. The present study aimed to investigate the association between BF and demographic and psychosocial variables in adults treated for a benign meningioma. Method: Seventy participants aged 36 to 87 years completed questionnaires that assessed perceived BF resulting from one's meningioma experience, intrusion and avoidance symptoms, emotional wellbeing and perceived quality of social support. Results: 39% of the sample had been diagnosed with their tumour within a 2-year period prior to assessment (‘early’ meningioma subgroup), while the remaining 61% of participants had been diagnosed with their tumour for more than 2 years (‘late’ meningioma subgroup). Almost two thirds (63%) of the sample reported deriving a moderate amount of BF due to their meningioma experience; although different psychological variables were found to be associated with BF according to group status. Whereas greater BF was found to be significantly associated with elevated depressive symptoms in the ‘early’ subsample, elevated BF was significantly positively correlated with intrusions and avoidance symptoms in the ‘late’ subsample. Conclusions: The findings indicate that BF may co-occur in context of both adaptive and positive illusory processes, which is consistent with the Janus-Face model of growth following adversity. The findings also support the use of psychosocial interventions to facilitate psychosocial adjustment following health crises.

The prevalence and nature of post-traumatic stress disorder (PTSD) following mild traumatic brain injury (MTBI) is controversial because of the apparent paradox of suffering PTSD with impaired memory for the traumatic event. In this study, 1167 survivors of traumatic injury (MTBI: 459, No TBI: 708) were assessed for PTSD symptoms and post-traumatic amnesia during hospitalization, and were subsequently assessed for PTSD 3 months later (N = 920). At the follow-up assessment, 90 (9.4%) patients met criteria for PTSD (MTBI: 50, 11.8%; No-TBI: 40, 7.5%); MTBI patients were more likely to develop PTSD than no-TBI patients, after controlling for injury severity (adjusted odds ratio: 1.86; 95% confidence interval, 1.78–2.94). Longer post-traumatic amnesia was associated with less severe intrusive memories at the acute assessment. These findings indicate that PTSD may be more likely following MTBI, however, longer post-traumatic amnesia appears to be protective against selected re-experiencing symptoms. There is active, current speculation about the relationship between trauma and psychosis. However, little is known about the information-processing mechanisms underlying the development of trauma-related intrusions in this area. Our account highlights the role of contextual integration, i.e. the need for experiential information to be effectively integrated into a temporal and spatial context in order to facilitate voluntary recall. Drawing on existing models of both posttraumatic stress disorder (PTSD) and psychosis (Brewin, 2001; Ehlers and Clark, 2000; Garety et al., 2001; Morrison, 2001), we propose a contextual integration account of trauma-related intrusions. It is argued that the strength of contextual integration, which occurs during encoding, influences the frequency and nature of subsequent intrusive experiences. Consequently, individual differences in schizotypal personality traits, which are known to be associated with levels of contextual integration, are also related to the phenomenology of trauma-related intrusions. Whilst intrusions can be seen to occur within a range of disorders, it is argued that contextual integration may be one key variable in understanding the relationship between an experienced trauma and any consequent psychiatric symptomatology. Implications for clinical interventions aimed at trauma-related psychosis are discussed, along with research aimed at developing the empirical basis for such interventions.

Structural and topographic relief along the eastern margin of the Rio Grande rift, northern New Mexico, provides a remarkable cross-section through the 26-Ma Questa caldera and cogenetic volcanic and plutonic rocks of the Latir field. Exposed levels increase in depth from mid-Tertiary depositional surfaces in northern parts of the igneous complex to plutonic rocks originally at 3–5 km depths in the S. Erosional remnants of an ash-flow sheet of weakly peralkaline rhyolite (Amalia Tuff) and andesitic to dacitic precursor lavas, disrupted by rift-related faults, are preserved as far as 45 km beyond their sources at the Questa caldera. Broadly comagmatic 26 Ma batholithic granitic rocks, exposed over an area of 20 by 35 km, range from mesozonal granodiorite to epizonal porphyritic granite and aplite; shallower and more silicic phases are mostly within the caldera. Compositionally and texturally distinct granites define resurgent intrusions within the caldera and discontinuous ring dikes along its margins; a batholithic mass of granodiorite extends 20 km S of the caldera and locally grades vertically to granite below its flat-lying roof. A negative Bouguer gravity anomaly (15–20 mgal), which encloses exposed granitic rocks and coincides with boundaries of the Questa caldera, defines boundaries of the shallow batholith, emplaced low in the volcanic sequence and in underlying Precambrian rocks. Palaeomagnetic pole positions indicate that successively crystallised granitic plutons cooled through Curie temperatures during the time of caldera formation, initial regional extension, and rotational tilting of the volcanic rocks. Isotopic ages for most intrusions are indistinguishable from the volcanic rocks. These relations indicate that the batholithic complex broadly represents the source magma for the volcanic rocks, into which the Questa caldera collapsed, and that the magma was largely liquid during regional tectonic disruption.

Volcanic and plutonic magmas (1) changed from early high-K calc-alkaline to alkalic prior to caldera eruptions; (2) differentiated to a weakly peralkaline rhyolite and equivalent acmiteartvedsonite granite cap (underlain by calc-alkaline granite) when the caldera formed at 26·5 Ma; then (3) reverted to calc-alkaline compositions. Concentrations of alkalis and minor elements such as Rb, Th, U, Nb, Zr, and Y reached maxima at the caldera stage. The volcanic rocks constitute intermittently quenched samples of upper parts of Questa magma bodies at early stages of crystallisation; in contrast, the comagmatic granitic rocks preserve an integrated record of protracted crystallisation of the magmatic residue as eruptions diminished. Multiple differentiation processes were active during evolution of the Questa magmatic system: crystal fractionation, replenishment by mantle and lower crustal melts of varying chemical and isotopic character, mixing of evolved with more primitive magmas, upper crustal assimilation, and perhaps volatile-transfer processes. As a result, an evolving batholithic cluster of coalesced magma chambers generated diverse assemblages of broadly cogenetic rocks within a few million years. Evolution of the Questa magmatic system and similar high-level Tertiary granitic batholiths nearby in the southern Rocky Mountains provides broad insights into magmatic processes in continental regions such as the overall shapes of batholiths, time and compositional relations between cogenetic volcanic and plutonic rocks, density equilibration of magmas with country rocks, and thermal evolution of continental crust.
