  • Pulmonary atresia and ventricular septal defect with major aortopulmonary collateral arteries is an extremely complex, heterogeneous, and rare anomaly. This group of patients may not be able to survive until adulthood without any interventions or treatment. Although surgical management of patients diagnosed in newborn, infant, or early childhood is clear, treatment of patients diagnosed in adulthood still remains a significant problem. The pre-operative clinical status, imaging methods, and operative findings might be helpful for planning the most appropriate management. Herein, we report a unique case of pulmonary atresia and ventricular septal defect with major aortopulmonary collateral arteries who remained asymptomatic until the age of 18 years.

  • Objective

    To evaluate VIIth–XIIth cranial nerve (hypoglossal–facial nerve) anastomosis results by age.

    Method

    A total of 34 patients who attended a follow-up visit in 2016, aged 20–63 years, were enrolled. The House–Brackmann facial nerve function grading system and the Facial Clinimetric Evaluation scale were applied.

    Results

    Regarding post-anastomosis facial nerve function, in the group aged 40 years or less, 14 patients (78 per cent) had House–Brackmann grade III and 4 patients (22 per cent) had House–Brackmann grade IV facial nerve function post-anastomosis. In the group aged over 40 years, nine patients (56 per cent) had House–Brackmann grade III and seven patients (44 per cent) had House–Brackmann grade IV facial nerve function post-anastomosis. There was a statistically significant difference between the two groups in mean facial movement domain scores (p = 0.02). Analysis between age and facial movement score in all 34 patients demonstrated a moderate negative correlation (Pearson correlation coefficient: −0.38) and statistical significance (p = 0.02).

    Conclusion

    Facial reanimation yielded better results in younger than in older patients.

  • Enhanced recovery programmes in head and neck surgery: systematic review

  • Objective:

    To review the literature on enhanced recovery programmes in head and neck surgery.

    Method:

    A systematic review was performed in May 2013.

    Results:

    Thirteen articles discussing enhanced recovery after laryngectomy, neck dissection, major ablative surgery and microvascular reconstruction were identified. Articles on general pre-operative preparation and post-operative care were also reviewed.

    Conclusion:

    Considerable evidence is available supporting enhanced recovery in head and neck surgery that could be of benefit to patients and which surgeons should be aware of.

  • Objectives:

    The purpose of the present study was to examine the clinical outcomes of using tracheoesophageal diversion for preventing intractable aspiration.

    Method:

    We retrospectively reviewed 25 patients who underwent tracheoesophageal diversion from 2003 to 2009 at our hospital (median age, 25 years; range, 0–78 years). End-to-side anastomosis was used in 16 cases and side-to-side anastomosis was used in 9.

    Results:

    The average operative time was 141 minutes for end-to-side anastomosis and 191 minutes for side-to-side anastomosis. Peri-operative complications were observed in only two (8 per cent) cases: one with infection and one with haematoma. No fistulas were observed. Aspiration was prevented in all cases, but the nutritional route depended on the swallowing function of the patient. Oral feeding was the main nutritional route after surgery in only four patients (16 per cent).

    Conclusion:

    This procedure is well suited to patients who lack speech communication and are at high risk of aspiration.
