Objective - To survey and analyse the extent of utilization of «Non Pharmacological Interventions» (NPI) in the public mental health services of a large city. Setting - Inteview of the entire medical staff employed in 1991 in 10 Departments of Mental Health in the urban area of Naples, Italy. Main measures - A «ad hoc» questionnaire was prepared to gather informations on prevalence of use on NPI in different therapeutic settings, the selected technique according to the different diagnostic categories and the degree of professional training of the interviewed medical personnel. Result - The NPI, as the only treatment, was preferred by the large majority (86%) of the medical staff. The patients treated with NPI was double in outpatient than in inpatient. Although the medical staff did not use the same diagnostic criteria (ICD-9, DSM-III, others) anxiety (49%) and personality disorders (14,3%) were the diagnostic categories most commonly selected for NPI. At the time of the survey 24% of the patients were treated with psychotherapy. Support psychoterapy, psychoanalysis, family therapy were the most frequently employed. The support psychotherapy was mainly used in treatment of schizophrenia. Near to 50% of the medical staff reported a combined use of up to five different NPI. By and large choice of NPI was found dependent on the type of training and inversly correlated with the degree of seniority. Conclusions - The data obteined from the present survey show the large use and variability in NPI employed in public mental health services. The analysis of psycotherapy used suggests a different approach between public and private sector.

The purpose of this research is to provide information about the state of psychotherapy in Argentina, as well as to study some variables involved in psychologists' evaluation of psychotherapy. A survey of 14 items was elaborated ad-hoc for this purpose, and administered to 226 psychologists from all over the country by mail and, in some cases, personally. Results suggest that the predisposition to apply techniques from different frameworks is associated with the amount of experience and with the perception of a bias concerning other theoretical focuses, although it is not related to the evaluation of the state of psychotherapy. On the other hand, in the view of psychologists, personal aspects of the therapeutic relationship are believed to be the most important factor in patients' change and effective progress.

This work presents a description of a psychotherapeutic group of psychosomatic patients conducted in an out-patient setting. We show the gains of insight in the group's organization and inter-personal communication, as well as the relief of symptoms in seven patients attended by this psychoanalytically oriented group therapy. We discuss the extent and the limits of the group technique for such patients and we conclude with some proposals about the efficacy of this therapeutic approach.

In this paper we present the adaptation of the Behavioral Activation for Depression Scale (BADS), developed by Kanter, Mulick, Busch, Berlin, and Martell (2007), in a Spanish sample. The psychometric properties were tested in a sample of 263 participants (124 clinical and 139 non-clinical). The results show that, just as in the original English version, the Spanish BADS is a valid and internally consistent scale. Construct validity was examined by correlation with the BDI-II, AAQ, ATQ, MCQ-30, STAI and EROS. Factor analysis justified the four-dimensions of the original instrument (Activation, Avoidance/Rumination, Work/School Impairment and Social Impairment), although with some differences in the factor loadings of the items. Further considerations about the usefulness of the BADS in the clinical treatment of depressed patients are also suggested.

Objective - The aim of the present paper is to assess the supply and demand of psychotherapy in mental health services and to identify the factors predicting the start of psychotherapy, using routine data, including 18 clinic and extraclinic variables, collected at 10 Mental Health Centers. Setting - Ten Mental Health Services of Regione Emilia-Romagna. Design - A prospective longitudinal study regarding all «new» users in 1995 followed for one year after the first visit. Results - The percentage of the new users starting a psychotherapy was 11.5%, corresponding to an overall one-year incidence of 5.6/10000 inhabitans ≥18 years old (range 0.9-14.8 across the 10 centres). The demand of psychotherapy in 1995 was 16.1%, including both the request of the patients and the primary care physicians (range 0.6-33%). The predictive factors for the beginning of the psychotherapy were: the presence of a psychologist at the first encounter, the request of the patient for a psychotherapy, age < 48 years, high educational level, being student, living with the family, the Mental Health Center; the broad diagnostic group of «neurotic» diagnoses resulted to have a twofold probability with respect to controls to start a psychotherapy. The logistic regression analysis of 18 variables coudn't find a predictive model with more than 33% sensitivity. Conclusions - The psychotherapeutic treatment is not predictable in the 10 Centers we studied, because of extreme differences in psychotherapy supply among Centers and probably because of uncertainty of therapeutic recommendations for psychotherapies.

Objective The purpose of this study is the systematic analysis of operators' points of view about psychotherapies concretely performed in the Public Psychiatric Public Services and inside the global operating mode of the assistance. Setting – The study has involved 26 CPS selected randomly in Lombardia. The sample has been built with 73 psychiatrists and 42 psychologists. Main outcome measures – All the therapeutists involved in this research have been submitted to a question-form querying socio-anagraphic data, professional training, orientations, operating modes in their CPS, rules and objectives of their psycotherapies, observations and evaluations about psychoterapic treatments and their effects. Results and conclusions: Psychiatrists and psycologists working in CPS, mostly the younger ones (less than 45 years old), followed a personal training in over the 70% of the cases. The prevalent orientation is the psycoanalityc one in both the categories. The most part of therapeutists deems that there has been an evolution in their way of conceiving psychiatry inside the public service. The lines at these evolutions have been mentioned explicitly in the article. Psychiatrists and psycologists, even with some concrete differences, seems to have mostly homogeneous points of view: psychiatrists have a more flexible vision of which practices can be considered as a psychotherapy, whereas psycologists are more rigorously linked to theoric reference models and to rules learnt during their training. They are both slightly favourable to the use of psychotherapies in their services, even if, as a matter of fact, they are used by just a few patients. No contrast between psycotherapy and psycopharmacology has been detected from operators' answers. These practices seem to be both considered useful and integrable.

Aims – Taking the scientific literature on psychotherapy as a starting point, the paper suggests a few remarks for the debate over psychological treatments in the mental health services in Italy. Methods – A survey of reviews and papers on process and outcome of psychological treatments and on psychotherapeutic practice in mental health services. Results – The paper examines some organizing hypotheses in order to increase psychological treatments supplies. Conclusion – The practicability of such solutions requires on the one hand more attention for empirically supported psychological treatments in mental health professionals training and, on the other hand, that various health services become able to be different in their treatments supply.
