Background: Sleep disturbance is a common phenomenon among opiate addicts. The side effects of opiate addiction or opiate withdrawal might result in sleep disturbance. However, their problems might be related to sedative medication abuse, alcohol abuse or heroin relapse. Sleep is an important issue in this population.

Objective: To evaluate the prevalence of sleep disorders in heroin addicts receiving methadone maintenance treatment (MMT) and analyse the correlation between related factors, such as age at opiate exposure, opiate exposure duration, duration in MMT, methadone current dosage, methadone attendance rate and the severity of sleep disorders.

Method: We enrolled 121 heroin addicts who were receiving MMT. We collected data on the duration of insomnia, hypnotic history, Visual Analogue Scale-10 of sleep quality, Pittsburgh Sleep Quality Index (PSQI), methadone dosage, methadone history and opiate history.

Results: The mean of the PSQI was 9.1 ± 5.4, and 70.2% of patients had PSQI scores >5, indicating they were poor sleepers. We also found the PSQI scores were correlated significantly with the methadone dosage.

Conclusions: The sleep disturbance prevalence rate of opiate addicts under MMT was high in Taiwan, as shown in the previous studies, and the severity of sleep disturbance has been underestimated.

Objective: Although reports in the literature describe deficits in working memory in borderline personality disorder (BPD), the evidence is limited and inconsistent. The aim of this study was to evaluate further this cognitive dimension and its clinical correlates in BPD.

Method: We compared the performance of 15 BPD patients to 1:1 matched healthy controls on verbal working memory as determined by the sequential letter N-back test and sustained attention as measured using the continuous performance test (CPT).

Results: BPD patients performed significantly worse on the N-back test compared to healthy controls (p < 0.05), but not on the CPT. The N-back deficit was more pronounced and significant in the 3-back condition and inversely correlated with impulsivity.

Conclusions: These results suggest the presence of working memory deficits in BPD that may be linked to greater impulsivity and sustained by impairment in the dorsolateral prefrontal cortex.

Bersani G, Clemente R, Gherardelli S, Bersani FS, Manuali G. Obstetric complications and neurological soft signs in male patients with schizophrenia.

Objective: The study investigated the relationship between neurological soft signs (NSS) and obstetric complications (OCs) in patients with schizophrenia.

Methods: Sixty-three male patients with schizophrenia were divided into two subgroups, based on the OCs presence or absence, which were compared in relation to NSS prevalence. After that, a Person's correlation test was performed to explore the correlation between NSS and OCs severity.

Results: The subgroup with OCs showed more NSS, but there were not significant correlations between NSS and OCs severity.

Conclusions: It seems that any OC, without distinction in typology and severity, could unspecifically impair the neurodevelopment and inducing NSS expression. Our findings confirm the hypothesis that neurodevelopment alterations, such as those probably induced by OCs, can contribute to a premorbid brain dysfunctional state expressed by NSS.

Objective: Caregiver behaviours and emotional expressions may induce development of two basic categories of constructing identity and of regulating cognitive and emotional processes: an Inward or an Outward Personal Meaning Organisation (PMO). Inwards read environmental signals through their internal activations. Their emotions are more distinct, and reciprocity is more based on physical distance (protection, loneliness). Outwards read internal activations through the environment. Their emotions are more blurred, and reciprocity is more based on a semantic sight of relations (approval, rules).

It has recently been shown that PMO development may also have physiological and genetic bases. In a functional magnetic resonance imaging (fMRI) study, Inward and Outward subjects showed different amygdala activation patterns and an association with the SLC6A4 serotonin transporter gene 5-HTTLPR polymorphism.

Methods: In this work, 149 healthy subjects were examined with respect to Inward and Outward PMOs. We explored the association with 10 serotonin receptor 2A (HTR2A) gene single nucleotide polymorphisms (SNPs) selected by bioinformatics methods.

Results: An intronic SNP (rs55948462) was found to be significantly associated with an Inward and an Outward PMOs development. However, after statistical adjustments, these results did not remain significant.

Conclusion: We did not find associations between considered SNPs and Inward/Outward PMOs. However, the role of HTR2A polymorphisms was not considered in this study and that of the other serotonin-related genes should be valued.

Chang H-A, Chang C-C, Chen C-L, Kuo TBJ, Lu R-B, Huang S-Y. Major depression is associated with cardiac autonomic dysregulation.

Objective: Altered cardiac autonomic function has been proposed in patients with major depression (MD), but the results are mixed. Therefore, analyses with larger sample sizes and better methodology are needed.

Methods: To examine whether cardiac autonomic dysfunction is associated with MD, 498 unmedicated patients with MD and 462 healthy volunteers, aged 18–65 years, were recruited for a case-control analysis. We used the Hamilton Depression Rating Scale (HAM-D) and the Beck Depression Inventory (BDI) to assess depression severity. Cardiac autonomic function was evaluated by measuring heart rate variability (HRV) parameters. Frequency-domain indices of HRV were obtained.

Results: Patients with MD exhibited reduced cardiac vagal control compared to healthy volunteers, and depression severity was negatively correlated with cardiac vagal control. Stratified analyses by suicide ideation revealed more pronounced cardiac vagal withdrawal among MD patients with suicide ideation.

Conclusion: This study shows that MD is associated with cardiac autonomic dysregulation, highlighting the importance of assessing HRV in currently depressed patients, given the higher risk for cardiac complications in these individuals. Taking into account that suicidal depressed patients had more adverse patterns of HRV, one might consider the treatment to restore the autonomic function for the patient population having increased susceptibility to autonomic dysregulation.

Gallerani M, Imberti D, Mari E, Marra A, Manfredini R. Risperidone and pulmonary embolism: a harmful association? Case series and review of the literature.

Objective: Risperidone is an atypical antipsychotic drug used to treat a number of psychiatric diseases, such as schizophrenia, schizoaffective and bipolar disorders and irritability in children with autism. Moreover, it is also often administered for short-term treatment of persistent aggression in people with moderate-to-severe Alzheimer's dementia. A possible association between risperidone and venous thromboembolism (VTE) has been described. We intended to verify the dimension of the problem in our hospital setting.

Methods: We considered all consecutive patients hospitalised in our Internal Medicine Department from January 2004 to December 2010, who were treated with risperidone and presented pulmonary embolism (PE).

Results: Four cases of patients, apparently free from the well-known major risk factors for VTE (i.e. cancer, prolonged immobilisation, acute cardiac and respiratory failure, infections), who presented PE associated with risperidone therapy, were reported in details.

Conclusions: A review of the available literature, discussing the possible different pathogenic reasons for this increased risk of VTE, is provided.

Urben S, Pihet S, Jaugey L, Halfon O, Holzer L. Computer-assisted cognitive remediation in adolescents with psychosis or at risk for psychosis: a 6-month follow-up.

Objective: To investigate short-term outcomes of a computer-assisted cognitive remediation (CACR) for adolescents with psychotic disorders or at high risk for psychosis.

Method: Cognitive abilities and clinical status were assessed at baseline (N = 32) and at 6-month follow-up (N = 22) after enrolment in either a CACR (treatment group) or a computer games (control group) program (8 weeks).

Results: With regard to the cognitive abilities, no amelioration was found in the control group while, in the CACR group, significant improvements in inhibition (p = 0.040) and reasoning (p = 0.005) were observed. Furthermore, symptom severity decreased significantly in the control group (p = 0.046) and marginally in the CACR group (p = 0.088). Improvements in cognitive abilities were not associated with symptoms' amelioration. Finally, increase in reasoning abilities was related to the median effective work time in sessions of CACR (R = 0.64, p = 0.024).

Conclusion: At follow-up, enhanced cognitive abilities (reasoning and inhibition), which are necessary for executing higher-order goals or adapting behaviour to the ever-changing environment, were reported in adolescents participants of the CACR. Thus, further studies are needed to confirm and extend these interesting results.

Kitaichi Y, Inoue T, Nakagawa S, Boku S, Koyama T. Effects of combined treatment with clorgyline and selegiline on extracellular noradrenaline and serotonin levels.

Objective Combined treatment with clorgyline, an irreversible monoamine oxidase (MAO)-A inhibitor, and selegiline, an irreversible MAO-B inhibitor, reportedly increases extracellular serotonin levels in the raphe nuclei more than clorgyline does alone. However, the effects of combination of these MAO inhibitors on extracellular noradrenaline have not been reported.

Methods Using in vivo microdialysis, we measured extracellular noradrenaline and serotonin levels after administration of clorgyline and/or selegiline in the medial prefrontal cortex of rats.

Results Administration of clorgyline (10 mg/kg) significantly increased both extracellular serotonin and noradrenaline levels. Combined treatment using clorgyline (10 mg/kg) and selegiline (3 mg/kg) increased extracellular serotonin and noradrenaline levels more than each drug alone did.

Conclusions These findings of this study suggest the augmented antidepressant action of the combination of MAO-A inhibition and MAO-B inhibition. The addition of a MAO-A inhibitor to selegiline or increasing dose of selegiline to achieve full MAO-A inhibition might be the promising strategy for the antidepressant treatment in partial responders or non-responders to selegiline.

Bharadwaj B, Sugaparaneetharan A, Rajkumar RP. Graves' disease presenting with catatonia: a probable case of encephalopathy associated with autoimmune thyroid disease.

Introduction: Encephalopathy associated with autoimmune thyroid disease (EAATD) is diagnosed when neuropsychiatric symptoms of acute or sub-acute onset occur along with clinical or subclinical autoimmune thyroid dysfunction. Supporting evidence includes the presence of anti-thyroid antibodies in the serum and/or cerebrospinal fluid. The thyroid hormone alteration is not sufficient to explain the neuropsychiatric manifestations. The most commonly described electroencephalographic abnormality in this condition is a generalised background slowing. Clinical descriptions of EAATD have focused mainly on neurological symptoms including seizures, loss of consciousness, myoclonus and cognitive symptoms.

Case: We present the case of a 48-year-old lady who presented with catatonia. Her clinical and laboratory features were suggestive of Graves' disease which was hitherto undiagnosed. Anti-thyroid antibodies were positive. Electroencephalogram showed a brief period of temporal delta activity. The findings are suggestive of EAATD. Differential diagnoses of metabolic derangements like electrolyte imbalance, hepatic and renal failure, neuroinfections and psychiatric conditions like acute psychosis were ruled out.

Discussion: Treatment involves a course of corticosteroids. Response to corticosteroids, however, is not essential for the diagnosis of EAATD as the patient may respond to symptomatic treatment alone as in our case. Outcomes reported in case series have been generally good as in our patient.

Conclusion: This report illustrates the need to keep a high index of suspicion for an organic aetiology in cases presenting with catatonia. A thorough neuropsychiatric evaluation is useful in such cases. In patients with severe neuropsychiatric manifestations associated with thyroid disease, an autoimmune thyroid disease should be considered as a possibility.

Takaki M, Okahisa Y, Kodama M, Mizuki Y, Sakamoto S, Ujike H, Uchitomi Y. Efficacy and tolerability of blonanserin in 48 patients with intractable schizophrenia.

Background: Blonanserin is effective for the treatment of schizophrenia in Korea and Japan.

Methods: We administered blonanserin to 48 Japanese patients with schizophrenia for whom other atypical antipsychotics were not sufficiently effective or tolerated.

Results: Previous antipsychotics were replaced with blonanserin because of its effectiveness (54.2%; 26/48) or tolerability (45.8%; 22/48). Blonanserin was more effective in 65.4% (17/26) of the and better tolerated in 95.5% (21/22) of the patients. Of 48 patients, 33 continued blonanserin for 1 year. The mean Clinical Global Impression of Severity scores improved from 4.60 to 2.48. The mean Global Assessment of Functioning score improved from 29.8 to 51.7. Nineteen patients (39.6%; 19/48) had a social role. The reasons for discontinuation of blonanserin were ineffectiveness against psychosis (27.1%; 13/48) or intolerability (4.2%; 2/48). The ratio of discontinuation for intolerability versus ineffectiveness was 0.15, which was the lowest among atypical psychotics.

Conclusions: Blonanserin may be effective and safe for the treatment of intractable schizophrenia.
