  • The role of pawnshops in risk coping in early twentieth-century Japan

  • This study examines the role of pawnshops as a risk-coping device in Japan in the early twentieth century, when the poor were very vulnerable to unexpected shocks such as illness. In contrast to European countries, Japanese pawnshops were the primary financial institution for low-income people up to the 1920s. Using data on pawnshop loans for more than 250 municipalities and exploiting the 1918–20 influenza pandemic as a natural experiment, we find that the adverse health shock increased the total amount of loans from pawnshops. This is because those who regularly relied on pawnshops borrowed more money from them than usual, and not because the number of people who used pawnshops increased. Our estimation results indicate that pawnshop loan amounts increased by approximately 7–10 percent due to the pandemic. These findings suggest that pawnshop loans were widely used as a risk-coping strategy.

  • Objectives:

    To describe school district preparedness for school closures and other relevant strategies before the coronavirus disease 2019 (COVID-19) pandemic.

    Methods:

    A stratified random sample of 957 public school districts from the 50 US states and the District of Columbia were surveyed between October 2015 and August 2016. The response rates for the questionnaires were as follows: Healthy and Safe School Environment, Crisis Preparedness Module (60%; N = 572), Nutrition Services (63%; N = 599), and Health Services (64%; N = 613). Data were analyzed using descriptive and regression techniques.

    Results:

    Most school districts had procedures that would facilitate the implementation of school closures (88.7%). Fewer districts had plans for ensuring continuity of education (43.0%) or feeding students during closure (33.8%). The prevalence of continuity of education plans was lower in the Midwest than the Northeast (adjusted prevalence ratio [aPR] = 0.68; 95% confidence interval [CI]: 0.51-0.90). Presence of plans for feeding students was higher in high-poverty than low-poverty districts (aPR = 1.41; 95% CI: 1.01-1.99) and in large districts than small districts (aPR = 2.06; 95% CI: 1.37-3.09).

    Conclusions:

    Understanding factors associated with having comprehensive emergency plans could help decision makers to target assistance during the current COVID-19 pandemic and for future planning purposes.

  • This article will argue that the memory of the great ‘Spanish’ influenza pandemic of 1918–19 played a significant role in the preparedness and response of Australia and New Zealand to the COVID-19 pandemic, and may help to explain their success compared with Europe and North America. An obvious alternative explanation for the success of Asian states such as China, Singapore, Hong Kong, South Korea, Taiwan, and Japan would be their experience of the SARS outbreak in 2002 and the H7N9 influenza outbreak of 2013. However, this explanation does not apply to Australia or New Zealand. All of these states had pandemic plans, initially developed with encouragement from the World Health Organization after the SARS outbreak, but only Australia and New Zealand appear to have directly incorporated ‘lessons’ from 1918–19 into their pandemic plans.

  • Summary

    This chapter highlights the impact of the war on women’s private everyday lives and explores how the wartime state increasingly reached into the home. It demonstrates how previously personal issues became political as women were urged to express their patriotism through their careful household management and by maintaining model homes and families for their absent husbands. The chapter also assesses the impact of the war on the standard of living of women in Ireland, interrogating previous interpretations of wartime prosperity and contrasting the urban and rural experiences. It explores the impact of the war on maternal and infant health, and the consequences of the 1918–19 influenza pandemic for women in Ireland. The chapter argues that the war resulted in much greater intervention of the state in women’s everyday and personal lives and brought significant hardship to many women. Far more women became reliant on governmental welfare through separation allowances, pensions and initiatives under the Prince of Wales National Relief Fund. Memoirs, diaries and letters are used to explore the experience of separated couples during the war and how women coped with the emotional hardship of the soldiers’ war service.

    During an influenza or coronavirus disease 2019 (COVID-19) pandemic that results in acute respiratory distress, the number of available ventilators will not meet demand. In 2007, the New York State Task Force on Life and the Law and Department of Health released draft Guidelines for ethical allocation of ventilators for adults. In 2015, updated guidelines were released to ensure that: (1) revisions reflect the public’s values and (2) the triage protocol is substantiated by evidence-based clinical data. We summarize the development and content of the 2015 Guidelines compared with the 2007 version, emphasizing new/revised aspects of the ethical considerations and clinical protocol.

    Methods:

    We compared the 2007 and 2015 guidelines, with particular emphasis on the ethical issues and clinical protocols.

    Results:

    The 2015 Guidelines retained much of the ethical and clinical framework of the 2007 draft. The triage protocol was revised using evidence-based clinical data. Patients with the highest likelihood of short-term survival with ventilator therapy have priority access. Protocol consists of exclusion criteria, the sequential organ failure assessment (SOFA) score, and periodic clinical assessments. Guidance is provided on secondary triage criteria. Other forms of medical intervention/palliative care and review of triage decisions are discussed.

    Conclusions:

    The 2015 Guidelines reflect advances in medicine and societal values and provide an evidenced-based framework to save the most lives. The framework could be adapted in other emergencies, such as the COVID-19 pandemic, that require ventilators.

  • The prospect of a severe influenza pandemic poses a daunting public health threat to hospitals and the public they serve. The event of a severe influenza pandemic will put hospitals under extreme stress; only so many beds, ventilators, nurses, and physicians will be available, and it is likely that more patients will require medical attention than can be completely treated. Triage is the process of sorting patients in a time of crisis to determine who receives what level of medical attention. How will hospitals sort patients to determine priority for treatment? What criteria will be used? Who will develop these criteria? This article formulates an answer to these questions by constructing a conceptual framework for anticipating and responding to the ethical issues raised by triage in the event of a severe influenza pandemic.
