Case 39 - Snoring and leg cramps

Summary

This chapter presents the clinical history, examination, follow-up, treatment, diagnosis, and the results of the procedures performed on a 57-year old patient who came to the sleep center complaining of snoring and of leg cramps. A polysomnography (PSG) study was performed with the sleep apnea protocol and video recording. The sleep study was predictably abnormal because of the presence of sleep apnea. The apnea-hypopnea index (AHI) overall was 33.2 events per hour and 59.4 while sleeping supine. Periodic breathing and Cheyne-Stokes respirations were also identified. The diagnosis was positional sleep apnea of moderate to severe intensity in the patient with primary lateral sclerosis, together with REM-sleep behavior disorder (RBD) by history. The patient was afflicted with primary lateral sclerosis, a slowly progressive noninherited gait disorder that may show no abnormalities on examination other than signs implicating the corticospinal tracts.
