Omnibus Christi fidelibus ad quos hoc praesens scriptum pervenerit Matthaeus, permissione divina Cantuariensis archiepiscopus, totius Angliae primas ac metropolitanus salutem in Domino sempiternam. Noveritis me praefatum Matthaeum ratione ad se valetudinis qua in praesentiarum maxime labore quominus praesenti hac sessione parliamenti interesse valeam nominare, ordinare et constituere dictos mihi et fideles in Christo R. Elien., N. London, permissione divina episcopos, meos veros certos et indubitatos factores, actores, attornatos seu procuratores per praesentes, dans et concedens eisdem procuratoribus meis coniunctim et divisim plenam auctoritatem et potestatem pro me et nomine meo de et super quibuscunque causis et negotiis in praesenti hac sessione parliamenti exponendis seu declarandis tractandi, tractatibusque huiusmodi inibi factis sive faciendis consilium et auxilium nomine meo imponendi, statutisque et ordinationibus quae ex communi maturo et deliberato iudicio dominorum tarn spiritualium quam [ 19] temporalium in praedicta sesione parliamenti congragatorum inactari seu ordinari contigerint, nomine meo consentiendi, ceteraque omnia et singula quae in praemissis necessaria fuerint seu quomodolibet requisita faciendi et exercendi in tarn amplis modis et forma ac ego ipse facere possem aut deberem, si praesens personaliter interessem; ratim et gratu habiturus totum et quicquid dicti procuratores mei coniunctim seu eorum aliquis divisim statuerit aut fecerit in praemissis. In cuius rei testimonium praesentibus subscripsi, sigillumque apposui.

Noveritis me praefatum N. episcopum antedictum ratione adversae valetudinis qua impraesentiarum detentus sum ac adeo gravatus et impeditus ut sine magno corporis mei periculo in instante parliamento dominae nostrae reginae apud Westmonasterium octavo die iam proxime futuri mensis Maii inchoando et tenendo interesse non possum sicut vellem et deberem, dilectos igitur mihi in Christo reverendos confratres meos R. episcopum W., N. episcopum W. et N. episcopum B., meos veros, certos et indubitatos factores, actores, attornatos sive procuratores per praesentes dans et concedens eisdem procurator [ibus] meis coniunctim et eorum cuilibet per se divisim plenam auctoritatem et potestatem pro me et nomine meo de et super omnibus et quibuscunque causis et negotiis in dicto instanti parliamento exponendis,
