Kidney disease is common in older age and associated with adverse health outcomes. Guidelines recommend estimating GFR (eGFR) using a standardized estimating equation that includes age, race, gender, and serum creatinine. Management for chronic kidney disease (CKD) includes identification and management of underlying causes to slow the progression, recognition, and treatment of CKD-related complications, and preparation for renal replacement therapy prior to end-stage renal disease. Despite the high burden of CKD in older adults, the evidence underlying existing guidelines comes primarily from young and middle-aged adults and often does not take into account multimorbidity, geriatric conditions, and heterogeneity in life expectancy and health goals at older age. A person-centered approach to CKD that includes geriatric assessment, engages and supports family caregivers, and incorporates palliative care principles across the spectrum of kidney disease is appropriate for older adults.
