Factors causing anxiety among healthcare professionals (HCP) are increasing, and psychosocial causes are the most common. During the COVID-19 pandemic, HCP are exposed to additional stressful factors.

The aim of this study was to assess the prevalence of trait-anxiety and its associated factors towards the COVID-19 outbreak among Tunisian HCP.

A cross-sectional descriptive and analytical study conducted among Tunisian HCP during November and December 2020. The data were collected by an online questionnaire including the sociodemographic information and the “General anxiety questionnaire of Spielberger” (STAI-Y-B) which was used to assess the trait-anxiety.

A total of 135 HCP participated in this study (47.4% female and 52.6% male). The average age was 31.98 years (SD=6.59). Of the participants, 3% were nurses, 8.1% were interns, 48.1% were residents, 34.8% were specialist doctors and 5.9% were generalist doctors. Concerning marital status, 61.5% were single, 36.3% were married and 2.2% were divorced. Of HCP involved in the study, 13.3% of the participants had a history of chronic somatic-disorder and 11.9 % of them had a history of a psychiatric disorder. The prevalence of trait-anxiety in HCP was 53.3%. The analysis showed that anxious HCP were more younger (p=0.002) and had history of a psychiatric disorder (p=0.017) compared to non-anxious HCP. However, no significant difference was found by gender, marital status and professional degree according to trait-anxiety.

In our study, we found that more than the half of HCP were anxious and these one were more younger and had history of psychiatric disorders.

Complete adherence to public health guidelines is essential to reduce the spread of COVID-19. Studies on the factors associated with increased/decreased adherence to these measures have the potential to inform public policies directed at increasing adherence, and thus helping to control the spread of the current pandemic.

This study aimed at assessing the demographic and psychosocial predictors of the perceived risk of the COVID-19 and adherence to confinement guidelines during the first mandatory lockdown in Portugal.

A convenience sample of 430 adults living in Portugal between March 19th and May 2nd, 2020 completed an online survey asking participants about the perceived risk of the COVID-19 and adherence to confinement guidelines. Participants also completed a sociodemographic questionnaire and measures of psychological function. Multiple regression analysis was performed.

Teleworking and Risk and COVID-19 controllability were significant predictors of the perceived risk of COVID-19 as measured by the perceived risk of being infected with COVID-19. Teleworking participants and those perceiving COVID-19 as less controllable reported a higher perceived risk of being infected with COVID-19 than those who were not in telework and perceived COVID-19 as a controllable condition. Adherence to confinement guidelines was predicted by the mental health status and perceived risk of COVID-19. Participants who reported worse mental health status, who perceived COVID-19 as a dangerous condition, and who trusted the public health system reported greater adherence to confinement guidelines.

The results of this study will be discussed considering their implications to public health policymaking to promote adherence to public health policies.

Clozapine is among the most effective antipsychotics used for treatment resistant schizophrenia. Adverse reactions to clozapine include neutropenia. Case series report that clozapine-treated patients with COVID-19 have no documented neutropenia.

We sought to investigate the potential adverse effect of coronavirus disease (COVID-19) in patients taking clozapine.

We retrospectively inspected data of 13 consecutive patients on clozapine, admitted to Highgate Mental Health Centre -Camden & Islington NHS Foundation Trust between March and June 2020. Selection was based on their COVID-19 symptoms presentation and/or COVID-19 positive test. We used a linear regression model with COVID status as independent variable and absolute neutrophil count (ANC) as dependent variable to inform about a correlation between COVID-19 status and neutrophil count. STATA was used for statistics.

We collected data on thirteen patients of which nine were male. The median age was of 41.97 years; six subjects were Black, three were Asian and four were White Caucasian. Ten subjects tested positive to COVID-19 and 3 were suspected cases -these latter were excluded from stastical analysis. During COVID-19 infection, neutrophils count (ANC) dropped significantly to 4.215 from a baseline value of 5.337. The beta values of 0.83 shows that ANC declined significantly during COVID-19 infection (p =<.0001, R2 = 95%). In three of thirteen patients, ANC drop was significant and changed the patients’ monitoring status from green to amber and required frequent blood tests.

Clinicians should bear in mind that a significant drop in neutrophils count may occur in COVID-19 -infected patients taking clozapine.

The COVID-19 pandemic affected today more than 76,000,000 worldwide, and more than half of humanity has been placed in quarantine. This pandemic affects mental health problems and influences the onset of symptoms.

The aim of this review is to analyze the impact of the COVID-19 pandemic on psychotic disorders and its interaction with the various risk factors.

We undertook a review of the impact of COVID-19 pandemic on psychosis. We carried out a systematic review of electronic databases using the keywords “COVID-19”, “pandemics”, “psychotic disorders”, and “delusions”. Relevant literature was limited to articles conducted around the world and published between January and December 2020.

We identified ten papers addressing incident cases of psychosis relapse linked to coronavirus pandemic. In multiple cases, psychotic symptoms were characterized by delusional thoughts about being infected by the coronavirus. The limited access to regular medications and psychosocial interventions was the main factor to psychotic relapse. This review included one cross-sectional clinical study comparing the impact of this pandemic on patients suffering from severe mental illness compared with healthy controls and they found that patients with mental disorders reacted to the pandemic and the lockdown restrictions with higher anxiety levels than the general public. Our study also revealed that elderly people suffering from psychosis and other chronic illness were the most vulnerable to relapse.

Psychotic disorders can relapse during stressful events like COVID-19 pandemic. Therefore, specific attention to these vulnerable subjects is crucial to prevent relapses in times of worldwide pandemic.

Both, the structure and function of COVID-19 have been documented and further research is in progress to fill the lacuna. Masses are hoping for a vaccine as their ultimate object of liberation. People are talking about crashing economy, but what about the crash of humans from their being? Here, a biological, psychological, and psychoanalytic understanding of COVID-19 is investigated. The impact of physical isolation has been documented, but mental isolation remains an uncharted space. The psychological trace—the paleontological psychic trauma of experiencing a pandemic as a witnessing subject is not much talked about. This effort is to open the paths to understanding COVID-19 which may seem pathless at first.

Primary sources are used along with an intradisciplinary and interdisciplinary unison of psychiatry, psychology, psychoanalysis, yoga, and meditation.

Yoga and meditation for strengthening both physical and psychological immunity along with facilitating the acceptance of psychological impact which is unregistered in the minds of a large population is elucidated.

Since February 2020, the outbreak of COVID-19 has spread to several countries worldwide, including Italy, leading to an uptake of telework.

We aim to evaluate the psychopathological impact of teleworking during the COVID-19 pandemic in Italy, identifying mental health determinants among home-based workers.

804 participants completed an online survey, including the psychometric scales “Depression, Anxiety and Stress Scale – 21 items” (DASS-21) and the “Insomnia Severity Index” (ISI). Teleworkers were also asked to provide information about their current work routine, home environment and clinical history.

At the DASS-21, 30% of the participants presented pathological levels of depression, 20.8% of anxiety and 30.7% of stress. At the ISI, 5% appeared to suffer from insomnia. Respondents with psychological and physical frailties, greater social isolation or inadequate working spaces manifested higher levels of psychiatric symptoms. Moreover, we also find a correlation of these symptoms with occupations in education. Telework was broadly appreciated and 87% of respondents expressed a willingness to maintain access to this arrangement.

Our results document that about a third of our sample manifested psychopathological symptoms while teleworking during the COVID-19 outbreak in Italy. However, telework itself does not seem to be directly associated with increased psychiatric symptoms, which were instead exacerbated by COVID-19-related stressful circumstances, as well as by constitutional and social determinants of health. Going forward, authorities should promote adequate measures in order to guarantee a healthy approach to teleworking.

The COVID-19 pandemic has challenged our model of face-to-face psychosocial treatment and rehabilitation format. To adapt to the current situation, as professionals, we have decided to transform the format into a virtual one that will offer the continuity of rehabilitation and therapy. Two clinical psychologists held online sessions and a special chat created in the IM messenger where patients could safely interact with each other.

This pilot study aimed to evaluate the effect of online sessions in a sample of outpatients engaged in rehabilitation programs.

Data from 50 patients (F20-F25, aged from 25 to 45) treated with a new online psychosocial program, including i. psychoeducation, ii. learning skills of the behavior under the circumstances of isolation, iii. training skills of effective communication and emotional regulation, and assessed for depression, anxiety, hopelessness, hostility (BDI, STAI, BHS, BDHI), and self-esteem, were analyzed for this study. Motivational enhancement techniques were also used to engage the patients in this new treatment format.

According to the preliminary data, we point out a statistically meaningful reduction in depression (p=0,003), anxiety (p=0,001), and hostility (р=0,001); self-esteem, evaluated with the Dembo-Rubinstein method, was improved (p=0,002); the T Wilcoxon criterion used for rating the magnitude.

Our results indicate that establishing a new online psychosocial program over the last few months positions us to respond effectively to such a new challenge and suggest that rehabilitative programs targeting patients’ needs may continue in this time of uncertainty.

Personality disorders comprise a set of diagnosis characterized by inflexible, pervasive and enduring patterns of cognition, affect, behavioural and social interaction. The status of research on the personality among different cultures implies the universality of traits and disorders, as well as, their measuresacross cultures.

In this retrospective case series, clinicaldata was collected from 40 patients who were hospitalized at the short-stay inpatient psychiatric service of the Príncipe de Asturias University Hospital during 2018.

Nineteen (47.5%) patients were European, ten (25%) were from South America, nine (22.5%) were Africanand two (5%) were Asian. Eight patients were diagnosed of diverse personality disorders. Seven (87.5%) of them were European, and only one (12.5%) was from South America.

This case series suggests various directions for future research. The fact that patients diagnosed with personalitydisorders were mainly European could indicate diverse conclusions. It would question the universality of personality disorders out of a Euro-american frame of reference. It would also point out the difficulty of diagnosing personality disorders, taking into consideration language, awareness of cultural values,traditions, interactional patterns, and social norms. More studies of traits and personality are needed, taking into account the culture and the society in which patients have grown and in which they currently live.

Ramadan happens in the ninth month of the Muslim lunar calendar. The cycle of the sun marks the beginning and the end of fasting. Its duration varies depending on the season: approximately 18 h in the summer to approximately 12 h during winter. The obligation to eat only during the night leads to an important change in the circadian rhythm There are certain psychiatric illnesses wherein people are very sensitive to this circadian disruption, bipolar disorder in particular. We know that a regulated circadian rhythm with adequate sleep are essential for symptom regulation and mood stability, with the risk of relapse or worsening symptoms. Additionally, some medications have to be maintained at a specific therapeutic index, namely lithium, a common mood stabilizer used to treat bipolar disorder.

All physiologic parameters are influenced by the circadian rhythm, which is influenced in its turn by the food rhythm. Studies on the effects of Ramadan on mood and mental health in the general population provide contradicting evidence. The inability to take medications during the day, dehydration and other somatic changes that necessitate dosing modification may lead to psychiatric symptom exacerbation.

Patients with bipolar disorder might be particularly sensitive to circadian rhythm disturbances and could require increased monitoring of their symptoms during this month.

Hwa-Byung is a unique syndrome based on social-cultural background of South Korea. However, the definition of Hwa-Byung has not been established. For example, Hwa-Byung Diagnostic Interview Schedule(Kim, Kwon, Lee & Park, 2004) and Hwa-Byung Scale(Kwon et al., 2008) that are generally used in clinical practices defined Hwa-Byung differently. According to this, there is a slight difference in the symptoms that are measured.

The purpose of our study is to establish the concept and symptoms of Hwa-Byung.

First, we review DSM-4, previous literatures and concept of Hwa-Byung in assessment tools. Through this, core features and characteristic symptoms are consisted. Second, a concept of Hwa-Byung that this study constructed is reviewed by clinical psychologists and Korean oriental psychiatrists. Finally, concepts and symptoms are defined.

Comprehensive definition of Hwa-Byung is established. Hwa-Byung was identified as a syndrome with symptoms that exploded in the form of anger because emotions such as anger could not be resolved. psychological symptoms include resentment, the baggage of mind, or a representative symptom of han. And physical symptoms include feeling heavy, heat, rush, lumps in the neck or chest. Finally, these physical and psychological symptoms are associated with distinct stressful events.

Our study defined the concept and categorized for physical and emotional symptoms of Hwa-Byung. This result suggests that it can contribute to the development and revision of the Hwa-Byung assessment tools.

A 31-year-old woman With no personal desease, developped a mutism, refusal of food and heteroagressiveness since the day after the wedding. The wedding party went well and it was consumed on the first night without any real incidents, yet, Ms. H was very anxious about the loss of her virginity and especially because of the low bleeding she had. The day after the wedding day, the bride was especially worried because of the presence of her family waiting in front of the bedroom to see the the blood-stained sheet : proof of the virginity of their daughter and the virility of the husband. In fact, incertain of the reaction of the family, the patient left her house early without informing her husband and was found by the police. Later, she developped an incoherent speech, audio-visual hallucinations and delusions against those around her.

In Tunisia, despite the progress made in terms of equality betwen men and women, women’s sexuality still suffer from certain taboos. Sexual education needs to be improved among young people to avoid subsequent sexual problems.

Globalization as lead to a more heterogeneous population than ever which makes intercultural communication an issue of modern times. Although this is positive in many ways, the differences in culture and beliefs, as well as a linguist barrier may impair clinical communication.

People from different cultural backgrounds have less access to health care and are less referenced to specialized care. Also, these patients report less satisfaction after their appointments. Although language proficiency in pointed as one of the most determinant factors, acceptance and comprehension of the patient beliefs regarding health and disease seems to play a very important role. Different cultures express symptoms differently and have different expectations when meeting a doctor. When these factors are overlooked, the doctor-patient relationship suffers and so does the treatment adherence. Doctors tend to have an identical approach to intercultural patients and native patients and to evaluate their interview as very positive, even when the same doesn’t happen with the patients.

Although the difficulties regarding intercultural communication are widely known, most doctors fail adequate their interventions to the specific needs of their patients, not taking into consideration their different beliefs and expectations. This raises very important questions as patient dissatisfaction leads to failure to report symptoms and consequent misdiagnosis and non-compliance to the proposed treatment which ultimately results in a less efficient health care in these populations.

Anti-NMDA-receptor encephalitis is a severe rare acute form of encephalitis caused by an autoimmune process with the synthesis of autoantibodies to the glutamate receptors. The average age of onser is estimated to be 23-25 years. A typical clinical picture consist of prodromal, psychotic, areactive, hyperkinetic phases, and a phase of gradual regression of symptoms. The disease usually lasts for a several weeks with spontaneous recovery or fatal outcome and caused by neoplastic process. Our case demonstrates that the course of anti-NMDAR encephalitis is possible at more mature age in the form of a long process with cultural features, without significant catadrome, inflammation and associated neoplastic process.

50-year-old Buddhist Buryat woman with complaints about hypomnesia, anosmia and dissomnia. The disease began with impaired consciousness, disorientation, seizures and memory loss 4 years ago. Then after a visit of Lama, the symtoms described above spontaneously reduced and schizophrenia-like psychosis developed (voice of Lama in the head, a belief in the existence of super-powers, disorganized behavior), mild neurological and severe neurocognitive symptoms, weight loss, intolerance to antipsychotics in minimal daily doses.

Examination included: CBC, metabolic panel, coagulogram, tumor markers, CSF, MRI, PET, specialists.

CBC, metabolic blood analysis, tumor markers - within the reference values. CSF: cytosis 9/3, glucose 5.5 mmol/l, Pandi++, Nonnet-Apeltau+, antibodies to the NMDA receptor - 8. MRI: signs of the consequences of encephalitis. PET: no signs of metabolic activity of the malignant process.

The diagnosis of anti-NMDA-receptor-encephalitis was confirmed by the laboratory and clinical criteria despite unususal course and age. The trigger role of herpes is possible, but not verified.

For individuals with mental disorders and their families, religion and spirituality may have a significant influence over how these conditions are understood, managed and treated. Family can act as a moderator in which psychotic patients interpret and explain internalized events. However, they can have a negative impact when discouraging diagnosis and treatment adherence.

Explore the impact of family religious and spiritual beliefs on clinical outcomes among a schizophrenic patient. Investigate the psychiatrist’s role in addressing barriers to treatment adherence.

We report a case of a 30-year-old single catholic woman, living with her parents. She had a past history of psychotic symptoms that were interpreted in a context of a depressive episode. After some months she fulfilled the criteria for Schizophrenia and anti-psychotic was prescribed. Family always demonstrated doubts about the disease and negatively influenced the treatment adherence. They believed she was possessed by demons and she was submitted to exorcisms and spiritual therapies. After a 2-year follow-up with erratic treatment regimens and worsening symptoms they accepted her hospitalisation. The majority of symptoms were controlled allowing complete adherence to the same treatment proposed before.

The disease acceptance is a complex process, influenced by multiple beliefs that play different roles in each patient and family, that can adversely influence clinical management. It is essential to understand the family sociocultural environment, by gauging the most influential elements aiming to enhance their compliance with treatment.

The regulation of emotions is a psychological process covering the capacity of inhibiting, maintaining or modulating emotions to create a coherent psychological functioning and to enable the individual to live in adequacy with his or her environment.

The aim of the study was to develop an Arabic version of the “Regulation of Emotions Questionnaire-2” (REQ-2T) from Phillips & Power, 2007, and to validate it in a sample of Tunisian university students.

This is a validation study conducted in a sample of 384 Tunisian university students to whom we administered the REQ-2T, the DASS-21 questionnaire and a sociodemographic questionnaire. We tested face and content validity, reliability, and construct validity of the Arabic version of the REQ-2.

Face and content validity were satisfying. The internal consistency was average, with Cronbach’s alpha coefficient ranging from 0.44 to 0.65. The inter-dimensional correlation reflected statistically significant and logical correlations within the REQ-2T. Temporal stability was satisfying with a variable concordance of 0.65. Exploratory analysis revealed four factors similarly to the original version of the questionnaire. Statistically significant correlations were found between the REQ-2T and its external validator (DASS-21).

The REQ-2T demonstrated good psychometric properties, thus this scale can be reliably used as a measure of emotion regulation in the Tunisian population.

Most people assume a direct connection between pathophysiology and clinical symptoms. However, evidence shows that the translation of pathophysiology and psychopathology into specific symptoms is mediated by cognitive processes and social interactions, which reflect models/practices specific to our culture. Patients focus on specific aspects of being sick, reinforced by cultural narratives or to fit expectations. Thus, people from different cultural backgrounds might have trouble communicating; cultural idioms of distress can be misinterpreted. The role of structural violence bestowed upon cultural minorities, which leads to discrimination and social exclusion, has extensively been studied as a risk factor for mental illness. Furthermore, ignoring cultural differences and diversity has been shown to contribute to healthcare disparities, hampering access to care and diminishing the quality of care received. In Canada, the Cultural Consultation Model provides cultural expertise, either by evaluating patients (preferably accompanied by the referring doctor, a translator and cultural mediator) for 1-3 sessions, providing recommendations to the referring doctor; or providing consulting to a referring doctor or organisation, through general guidance or discussion of specific cases.

In an evergrowing globalization process, we will inevitably have more contact with patients from culturally distinct backgrounds. To provide the best care, we must be aware of the ways in which culture can shape symptom expression, and take into account cultural explanations and preferences.

The article is devoted to the main methodological problems of qualitative research in the psychology of culture. Methods of ethnographic (field) observation were borrowed by psychology from social and cultural anthropologists in the first half of the twentieth century, and in modern research practices there is a special ethnographic direction that claims to analyze behavior and lifestyle in various subcultures and communities.

The purpose of this study was to identify the key cross-cultural difference in coping behavior of respondents from Moscow and Tashkent.

The methodological basis of the research was the thematic analysis of narratives and free-form interviews. The sample was N = 60 (residents of Russia and Uzbekistan age 17-39).

The results of thematic analysis of narratives and free-form interviews of respondents from Moscow and Tashkent allow us to conclude that the key cross-cultural difference in coping behavior is the degree of its individualization: representatives of Uzbek culture are focused on receiving support and care from significant Others, and not on independent internal work (unlike Russian respondents). At the same time, they are not satisfied with the traditional prescriptions that come from the family environment, which forces them to coping practices that go beyond the boundaries of normative social (often religious) ideas.

This can be interpreted from the point of view of the process of modernization of Uzbek culture, which is gradually becoming individualistic, and the latter circumstance requires the construction of flexible coping strategies in the situation of social and cultural changes.
