Ways in which chemical techniques could be applied to the understanding of neural systems, their functioning and their disorders were devised only gradually during the present century. In a particularly successful procedure, now termed assay-guided isolation, neural defects were made good by means of tissue-extracts and the restoration of function was established as an assay-system to guide the chemical separation and identification of the active tissue constituent. Thiamin was so isolated, using an experimental polyneuritis assay; subsequent instances among other metabolites, hormones, neurotransmitters and nerve growth factors are recounted. Procedures of assay-guided characterization ensured that links were retained between specific, sparsely-occurring substances and chosen aspects of their biological roles while their chemical nature was first explored and then established. The procedures discouraged the too-facile postulating of hypothetical molecules and contributed to the distinctiveness of neurochemistry as a subject within the neurosciences.

In the mid-nineteenth century opium and its derivatives, such as laudanum and morphine, were the most common poisons in suicides in England and Wales. With legislative restrictions on these ‘dangerous drugs’ such a use declined. This study attempts to show this trend and indicates the large variety of these opium-related suicides.

In 1903 Pierre Janet published perhaps the first detailed descriptions of patients with bulimia. Janet's observations, although anecdotal, are consistent with modern studies suggesting an association between bulimia and major affective disorder and between bulimia and anxiety disorders. Janet's writings also support the belief that bulimia is more prevalent today than a century ago.

Using computed tomography, lateral ventricular size was studied in a sample of 112 institutionalized chronic schizophrenic patients (selected from 510 cases to investigate the correlates of the defect state and intellectual decline and the effects of insulin, electroconvulsive and neuroleptic treatment), and compared with matched groups of non-institutionalized schizophrenics, patients with first schizophrenic episodes, institutionalized and non-institutionalized patients with primary affective disorder, and neurotic out-patients. Age was significantly correlated (P < 0·0002) with lateral ventricular size, but the institutionalized schizophrenic patients had significantly larger (P < 0·025) lateral ventricles than the neurotics when age was taken into account. Ventricular enlargement was unrelated to past physical treatment (neuroleptics, insulin coma and electroconvulsive therapy).

Within the group of institutionalized schizophrenic patients few correlates of ventricular enlargement were identified; thus in this population increased ventricular size was not clearly associated with the features of the defect state (negative symptoms and intellectual impairment). however, there was a curvilinear (inverted-U) relationship between intellectual function and ventricular size was significantly related to absence of hallucinations, impairment of social behaviour, inactivity and the presence of abnormal involuntary movements.

The findings confirm that structural brain changes do occur in chronic schizophrenia, but illustrate some of the difficulties in elucidating the clinical significance of ventricular enlargement. Lateral not bimodal; the relationship to particular features of the disease is complex and likely to emerge only in studies with a large sample size.

A sample of 77 women of high and low noise sensitivity, living in areas of high and low exposure to aircraft noise drawn from the 1977 West London Survey, were interviewed in the community in 1980. Women of high, intermediate and low noise sensitivity in 1980 were compared for measures of psychiatric disorder, personality and reactivity to the other sensory stimuli. In addition, the noise sensitivity measures defining the three groups in 1980 were compared with further measures of noise sensitivity. High noise sensitive women exhibited significantly more psychiatric symptoms, higher neuroticism scores, and greater reactivity to other sensory stimuli than intermediate and low noise sensitive women.

This article discusses the trial of a woman accused of murder in 1890 whose defence rested on the claim that she acted unconsciously under the hypnotic influence of her older lover. This relatively banal case brought together two rival schools of French psychiatry – that of J.-M. Charcot in Paris and that of Hippolyte Bernheim in Nancy – and provided a wide-ranging examination of views on the nature of unconscious mental activity as well as the social, political and professional implications that their theories on hypnotism and hysteria contained. Discussions on women's sexuality, family relations, crowd behaviour and political radicalism all played a part in the debate and are examined through the case study that the trial of Gabrielle Bompard permits. Moreover, the trial shed incidental light on the campaign by physicians against amateur healers and hypnotists whom they blamed for unleashing a wave of mass hysteria through their theatrical representations. The episode was one important element in the struggle for the passage of the law of 30 November 1892, which outlawed amateur practitioners and established the medical monopoly over healing in France.

The disorders of affect have not contributed much to the diagnostic definition of mental disease, and their phenomenological description has never achieved the richness of the psychopathology of perception or cognition. This paper shows how the subordinate role played by affectivity in the Western concept of man led to the early and enduring view of mental illness as an exclusive disturbance of intellect. Attempts by nineteenth-century alienists to challenge this notion were only partially successful, due to the conceptual unmanageability of most forms of affective behaviour and the terminological redundancy that this engendered. These efforts were frustrated by the rebirth of Associationism, the rise of brain localization experiments, the peripheralist definition of the emotions and, finally, by the unfolding of Darwinism. As a result, no autonomous psychopathology of affectivity was ever developed. The eventual recognition of the so-called ‘primary’ disorders of mood has not led, however, to a refinement in the semiology of the experiences themselves. This has been impeded by the use of descriptive behavioural surrogates or by metapsychological accounts of affect as a form of energy or as a driving force. None of these developments has contributed to the clinical description of the mood disorders.
