Recent findings suggest that neural representations of semantic knowledge contain information about category, modality, and attributes. Although an object’s category is defined according to shared attributes that uniquely distinguish it from other category members, a clear dissociation between visual attribute and category representation has not yet been reported. We investigated the contribution of category (living and nonliving) and visual attribute (global form and local details) to semantic representation in the fusiform gyrus. During functional magnetic resonance imaging (fMRI), 40 adults named pictures of animals, tools, and vehicles. In a preliminary study, identification of objects in these categories was differentially dependent on global versus local visual feature processing. fMRI findings indicate that activation in the lateral and medial regions of the fusiform gyrus distinguished stimuli according to category, that is, living versus nonliving, respectively. In contrast, visual attributes of global form (animals) were associated with higher activity in the right fusiform gyrus, while local details (tools) were associated with higher activity in the left fusiform gyrus. When both global and local attributes were relevant to processing (vehicles), cortex in both left and right medial fusiform gyri was more active than for other categories. Taken together, results support distinctions in the role of visual attributes and category in semantic representation. (JINS, 2009, 15, 169–181.)

Previous studies of schizophrenia have suggested a linkage between neuropsychological (NP) deficits and hippocampus abnormality. The relationship between hippocampus volume and NP functioning was investigated in 24 patients with chronic schizophrenia and 24 matched healthy controls. Overall intracranial, white and gray matter, and anterior (AH) and posterior (PH) hippocampus volumes were assessed from magnetic resonance images (MRI). NP domains of IQ, attention, and executive function were also evaluated with respect to volumetric measures. It was hypothesized that AH and PH volumes and episodic memory scores would be positively associated in controls and that the schizophrenia group would depart from this normative pattern. NP functioning was impaired overall and AH volume was smaller in the schizophrenia group. In the controls, the hippocampus–memory relationships involved AH and PH, and correlations were significant for verbal memory measures. In the schizophrenia group, positive correlations were constrained to PH. Negative correlations emerged between AH and verbal and visual memory measures. For both groups, cortical volume negatively correlated with age, but a negative correlation between age and hippocampus volume was found only in the schizophrenia group. In this sample of adults with schizophrenia, atypical relationships between regional hippocampus volumes and episodic memory ability were found, as was an atypical negative association between hippocampus volume and age. Past research has been inconsistent with regard to the effects of normal aging and sex on strategy use during verbal fluency performance. In the present study, both Troyer et al.’s (1997) and Abwender et al.’s (2001) scoring methods were used to measure switching and clustering strategies in 60 young and 72 older adults, equated on verbal ability. Young adults produced more words overall and switched more often during both phonemic and semantic fluency tasks, but performed similarly to older adults on measures of clustering. Although there were no sex differences in total words produced on either fluency task, males produced larger clusters on both tasks, and females switched more frequently than males on the semantic but not on the phonemic fluency task. Although clustering strategies appear to be relatively age-insensitive, age-related changes in switching strategies resulted in fewer overall words produced by older adults. This study provides evidence of age and sex differences in strategy use during verbal fluency tests, and illustrates the utility of combining Troyer’s and Abwender’s scoring procedures with in-depth categorization of clustering to understand interactions between age and sex during semantic fluency tasks. We sought to identify whether deficits in selective attention are present in pediatric brain tumor patients. Selective attention was assessed with covert-orienting, filtering, and visual-search tasks in 54 patients with either (1) posterior fossa (PF) tumors treated with cranial radiation and surgery (n = 22); (2) PF tumors treated with surgery alone (n = 17); or (3) non-CNS tumors (n = 15), who served as a patient control group. To account for normal development, patient performance was also compared with that of healthy age-matched controls (n = 10). We found that in PF tumor patients selective attention was impaired, regardless of whether they were treated with cranial radiation and surgery or surgery alone. However, patients treated with cranial radiation were most impaired. These patients may have greater damage to posterior brain regions know to mediate selective attention as the result of tumor location, effects of surgery, and higher doses of radiation to the posterior regions of the brain. These findings help to elucidate the potential impact of pediatric brain tumors and their treatment on discrete attentional skills. (JINS, 2009, 15, 205–216.)

Epidemiological studies suggest that the Hispanic population is at increased risk for neurological disorders. Yet, few assessment measures have been developed for, adapted to, or normalized with Spanish-speakers. The Neuropsychological Screening Battery for Hispanics (NeSBHIS) was developed to address the lack of resources available to this underserved community. Although the NeSBHIS possesses robust construct validity and clinical utility in a community-based sample, these properties remain largely untested in neurological populations. One hundred and twenty-seven Spanish-speaking Hispanic patients with confirmed epilepsy (mean age = 37.8, SD = 13.3) were evaluated using the NeSBHIS. All participants self-identified as “Hispanic” and immigrated from Spanish-speaking countries. Data were analyzed using confirmatory factor analysis with the a priori assumption that variables would load according to theoretical expectations reported by Pontón and colleagues (2000). The overall model fit indices were in the desired range: Comparative Fit Index = 0.936, Tucker Lewis Index = 0.915, RMSEA = 0.090, and SRMR = 0.069. All NeSBHIS subtests loaded significantly (p < .001) on their respective factors; the standardized loadings were high, ranging from 0.562 to 0.995, with the exception of Block Design (–0.308). Overall, findings suggest that the NeSBHIS has robust construct validity in a neurological sample. (JINS, 2009, 15, 217–224.)

Neurologically normal people tend to collide with objects on the right side more frequently than with objects located on the left side of space. This phenomenon could be attributable to pseudoneglect wherein individuals selectively attend to the left field. The current study investigated this effect using a virtual route-following task that was presented centrally, in the lower field, and in the upper field. Handedness was also examined. Fifty-two participants (four left handed) completed this task, and when presented in the lower field, more left-side collisions emerged. In the upper condition, this bias reversed direction to the expected rightward bias. In the central condition, there was no significant directional bias in collision behavior. An interaction between handedness and presentation condition indicated that left-handed participants experienced more right-side collisions in the central condition. Collectively, these results suggest that directional biases (i.e., left vs. right) in collision behavior are modulated by both location in the visual field (central, upper, or lower) and handedness. While it is well known that picture naming (PN) is impaired in Alzheimer’s disease (AD), sound naming (SN) has not been thoroughly investigated. We postulated that SN might be impaired more severely and earlier than PN, given the early involvement of the temporal cortex by AD-related pathology. SN and PN were assessed in 21 normal participants, 40 patients with mild cognitive impairment (MCI), and 27 patients in early stages of AD. Our results showed that SN accuracy and latency were more sensitive to advancing pathology in AD than PN accuracy and latency. SN was more useful and specific in distinguishing MCI patients from normal participants and therefore in potentially identifying the subset of MCI patients who already have impairment in more than one cognitive domain and may actually have incipient AD. These findings indicate a potential diagnostic utility of SN for early detection of the disease. Furthermore, even though most AD patients demonstrated more or less comparable impairment in both tasks, some were disproportionately impaired on SN and others were differentially impaired on PN. Future studies may be able to show that these discrepant groups correspond to patients with right and left hemisphere predominant AD, respectively. (JINS, 2009, 15, 231–238.)

The objective of this study was to examine the unique contribution of social cognition to the prediction of community functioning and to explore the relevance of social cognition for clinical practice. Forty-six schizophrenia patients and 53 healthy controls were assessed with tests of social cognition [emotion perception and Theory of Mind (ToM)], general cognition, and, within the patient sample, psychiatric symptoms. Community functioning was rated by nurses or family members. Social cognition was a better predictor of community functioning than general cognition or psychiatric symptoms. When the contributions of emotion perception and ToM were examined separately, only ToM contributed significantly to the prediction of community functioning. Independent living skills were poor in patients with impaired social cognition. In controls, social cognition was not related to community functioning. ToM was the best predictor of community functioning in schizophrenia. However, to fully understand a patient’s strengths and weaknesses, assessment of social cognition should always be combined with assessment of general cognition and psychiatric symptoms. Although some evidence exists that child molesters may be characterized by structural and functional brain abnormalities, findings across studies are inconsistent. Past cognitive research in this area has been extensively criticized for relying on conceptually weak batteries, measures of questionable reliability, and poorly defined samples (i.e., failing to distinguish between pedophilic and nonpedophilic child molesters). The present study aimed to address the weaknesses of past research by comparing 40 child molesters (20 pedophilic and 20 nonpedophilic) and 20 demographically matched nonoffender controls on six well-defined neurocognitive composite scores of comparable reliability (i.e., semantic knowledge, executive functioning, processing speed, motor speed, auditory memory, and visual memory). Results indicated that pedophilic child molesters exhibit slower processing speed, nonpedophilic child molesters exhibit poorer semantic knowledge, and both molester groups exhibit executive weaknesses as compared to nonoffender controls. This study is the first to compare the two molester types on neurocognitive functions. The observed differences between the molester groups help explain inconsistencies in past research and demonstrate the need to distinguish between the two types of child molesters when studying neurobiologic underpinnings of sexual offending. Financial capacity is a complex instrumental activity of daily living critical to independent functioning of older adults and sensitive to impairment in patients with amnestic mild cognitive impairment (MCI) and Alzheimer’s disease (AD). However, little is known about the neurocognitive basis of financial impairment in dementia. We developed cognitive models of financial capacity in cognitively healthy older adults (n = 85) and patients with MCI (n = 113) and mild AD (n = 43). All participants were administered the Financial Capacity Instrument (FCI) and a neuropsychological test battery. Univariate correlation and multiple regression procedures were used to develop cognitive models of overall FCI performance across groups. The control model (R2 = .38) comprised (in order of entry) written arithmetic skills, delayed story recall, and simple visuomotor sequencing. The MCI model (R2 = .69) comprised written arithmetic skills, visuomotor sequencing and set alternation, and race. The AD model (R2 = .65) comprised written arithmetic skills, simple visuomotor sequencing, and immediate story recall. Written arithmetic skills (WRAT-3 Arithmetic) was the primary predictor across models, accounting for 27% (control model), 46% (AD model), and 55% (MCI model) of variance. Executive function and verbal memory were secondary model predictors. The results offer insight into the cognitive basis of financial capacity across the dementia spectrum of cognitive aging, MCI, and AD. (JINS, 2009, 15, 258–267.)

This study examined whether patients with mild cognitive impairment (MCI) who are at higher risk for later Alzheimer disease (AD) display deficits comparable to patients with diagnosed dementia. We assessed 27 patients with MCI, 36 patients with AD, and 20 healthy older adults with an emotional variant of the Deese–Roediger–McDermott-paradigm. Participants studied four lists that were semantically related to a nonpresented critical theme word. These theme words were either depression-related (i.e., loneliness) or delusion-related (betrayal) or had a positive (holidays) or neutral (window) valence. Despite a normal overall emotional memory and a normal corrected overall false recognition, patients with MCI, as predicted, produced as many false memories as patients with AD. On closer examination, both patient groups showed enhanced false memories to unrelated stimuli and a significant bias to falsely remember stimuli with a positive valence. We conclude that although patients with MCI are not distinguishable from healthy older adults in terms of their overall emotional recognition, positively valenced memories and more specifically false positive memories may represent the signature of a breakdown of emotional memory along the continuum between normal aging and AD. Multiple sclerosis (MS) is a white matter disease associated with neurocognitive difficulties. More recently the potential for white matter pathology to also disrupt important aspects of emotion understanding has been recognized. However, no study to date has assessed whether capacity for facial affect recognition and theory of mind (ToM) is disrupted in MS, or whether any observed deficits are related to more general cognitive impairment. In the present study MS participants (n = 27) and nonclinical controls (n = 30) were administered measures of facial affect recognition, ToM, and cognitive functioning. MS participants were significantly impaired on the ToM task, and also presented with specific deficits decoding facial emotions of anger and fear. Performance on the measures of facial affect recognition and ToM were related to general cognitive functioning, and in particular, measures sensitive to executive dysfunction and information processing speed. These data highlight the need for future research to more fully delineate the extent and implications of emotion understanding difficulties in this population. (JINS, 2009, 15, 277–285.)

We investigated the neuropsychological profile of subjects in a subclinical obsessive-compulsive disorder (OCD) sample. Psychometrically defined subclinical obsessive-compulsive (n = 21) and control (n = 22) subjects were examined. Comprehensive neuropsychological tests evaluating verbal/nonverbal memory, attention, and executive function were administered. The subclinical obsessive-compulsive group showed poorer performances on the Wisconsin Card Sorting Test (WCST), F(1, 41) = 13.80, p < .001, and Trail-Making Test (TMT), F(1, 41) = 5.48, p < .05, compared with the control group. The subclinical obsessive-compulsive group showed higher rates of total errors, perseverative errors, and perseverative responses. In addition, the subclinical obsessive-compulsive group committed a greater number of errors in the TMT. However, the groups showed no performance differences in the TMT after controlling for the effects of depression and anxiety, F(1, 39) = 0.11, p = .739. These results suggest that subclinical obsessive-compulsives seemed to display deficits in executive functioning. This neuropsychological profile is consistent with current theories proposing that executive dysfunction may serve as the pathophysiological mechanism underlying the development of obsessive-compulsive disorder. The purpose of this study was to evaluate longitudinally, using the Iowa Gambling Task (IGT), the dynamics of decision-making capacity at a two-year interval (median: 2.1 years) in a group of patients with multiple sclerosis (MS) (n = 70) and minor neurological disability [Expanded Disability Status Scale (EDSS) ≤ 2.5 at baseline]. Cognition (memory, executive functions, attention), behavior, handicap, and perceived health status were also investigated. Standardized change scores [(score at retest-score at baseline)/standard deviation of baseline score] were computed. Results showed that IGT performances decreased from baseline to retest (from 0.3, SD = 0.4 to 0.1, SD = 0.3, p = .005). MS patients who worsened in the IGT were more likely to show a decreased perceived health status and emotional well-being (SEP-59; p = .05 for both). Relapsing rate, disability progression, cognitive, and behavioral changes were not associated with decreased IGT performances. In conclusion, decline in decision making can appear as an isolated deficit in MS. (JINS, 2009, 15, 291–295.)

Cancer patients frequently report cognitive complaints following chemotherapy, but the results from the available studies, mainly of women with breast cancer, are inconsistent. Our aim was to compare cognitive function of men with testicular cancer (TC) who had orchiectomy and chemotherapy (bleomycin, etoposide, cisplatin) with men who had orchiectomy only or orchiectomy and radiotherapy. Thirty-six chemotherapy patients and 36 nonchemotherapy patients were tested 2–7 years after treatment for TC with standardized neuropsychological tests. Chemotherapy and nonchemotherapy patients displayed similar performances on cognitive tests (p values adjusted for multiple comparisons: .63–1.00). Moreover, there was no difference in the proportion of cognitively impaired patients in the chemotherapy group (5.6%) compared to the nonchemotherapy group (8.3%) (χ2 = 0.22, p = .64). Our results are discordant with previous findings indicating cognitive impairment following chemotherapy and suggest that TC patients do not need to fear long-term cognitive consequences following chemotherapy. Shared vulnerabilities have been described across disorders of impulse control, including pathological gambling (PG) and bulimia nervosa (BN). Our aim was to compare the executive functioning of PG and BN females in order to confirm their similarity at a neurocognitive level. A total of 15 BN females, 15 PG females, and 15 healthy control (HC) females were administered the Wisconsin Card Sorting Test (WCST) and the Stroop Color and Word Test. Analysis of covariance adjusted for age and education was conducted to compare groups. PG showed the greatest impairment, that is, the highest percentage of WCST perseverative errors (p = .023), the lowest percentage of conceptual-level responses (p = .034), and the highest number of total trials administered (p = .021), while BN showed the highest percentage of WCST nonperseverative errors (p = .003). Both BN and PG females demonstrated executive dysfunction relative to HCs but different specific correlates (i.e., greater vulnerability to distraction in BN, but more cognitive inflexibility in PG). Six individuals with probable Alzheimer’s disease (AD) participated in a phase 1 study employing a repeated measures, parallel baseline design testing the hypothesis that error-free experience during word production practice combined with an acetyl cholinesterase inhibitor would improve confrontation naming ability. While acetyl cholinesterase inhibitors are safe and delay cognition decline associated with AD, improvement over baseline cognition is less evident; clinically significant cognitive deficits persist and progress. Both animal and clinical research strongly implicate acetylcholine in learning, a form of neuroplasticity. In clinical practice, however, people with AD are given cholinergic medications without concomitant systematic/targeted retraining. In this study six participants with probable AD and taking donepezil participated in targeted word production practice using an errorless learning strategy. Results showed that combining behavioral enrichment training and an acetyl cholinesterase inhibitor resulted in significant improvements in verbal confrontation naming of trained items for three of six participants. Differences in baseline dementia severity, living conditions, and medications may have influenced the training response. Detection of substantial treatment effects in 50% of subjects suggests further language treatment studies in AD in combination with an acetyl cholinesterase inhibitor are warranted and provide useful information on inclusion/exclusion criteria for use in subsequent studies.
