Studies have investigated the prevalence of compassion satisfaction and compassion fatigue in various healthcare professions. However, the majority of evidence is linked to the nursing profession and little is known about paramedical professions such as radiography and even less is known about its prevalence in students. The purpose of this study was to describe the levels of compassion satisfaction and compassion fatigue in the student population and how they varied in time.

Students undertaking radiotherapy training at the researcher’s host sites were surveyed using the Professional Quality of Life questionnaire at the end of each final clinical block in each year of their training.

During the 3 years of training compassion satisfaction falls and burnout increases in the student population, although the change is not significant. Secondary traumatic stress increases significantly during the 3 years of training, F=5·725, p=0·005. Considerable variation also exists in the three scores dependent on the student’s clinical training site. Relationships are also observed between some personality traits, particularly conscientiousness and neuroticism and compassion scores.

Evolving data are showing the need of considering smaller planning target volume (PTV) margin in paediatrics compared with adults treated for the same body site. This study proposed to evaluate the current patterns of practice regarding the PTV margin in paediatric patients compared with adult patients through an international survey.

A four-item questionnaire was created to address the PTV margins for paediatrics and adults as part of a comprehensive survey. International Paediatric Radiation Oncology Society (PROS) members were selected to partake and were contacted via email.

In total, 43 responded to the survey. The majority of the responders have written guidelines for PTV margin while the majority of those who have guidelines do not have separate guidelines for paediatrics. The implemented PTV margin for paediatric patients was in the majority 3–5 mm for the head region and 5–10 mm for the torso region and the difference from the PTV margin implemented in adults was not statistically significant.

The majority of responders employ a series of site-specific PTV margin protocols that are applied to both adults and paediatrics, and do not take into consideration patient age or size. These results highlight the need of a separate policy for PTV margin in adults and paediatrics in every institution.

During the treatment of breast cancer, radiotherapy to the supraclavicular fossa region results in absorption of radiation by the thyroid gland and consequently leads to hypothyroidism in 40% of patients. The aim of this study was to compare thyroid gland radiation absorption during radiotherapy with different anterioposterior beam radiation of 6–15 and 15–15 MV photon beam energies.

In total, 29 patients with breast cancer were recruited to this study. Adjuvant radiotherapy with a total dose of 50 Gy was performed for each participant. Thyroid gland dosimetric measurements were evaluated including, mean dose, minimum and maximum dose, and V20, V30, V40 and V50 (percentage of thyroid volume receiving ≥20, ≥30, ≥40 and ≥50 Gy, respectively). The irradiation delivered doses were measured using Prowess Panther treatment planning system (Version 5.5). All data were evaluated using SPSS software.

In total, 29 subjects with mean age of 53·4±9·4 were studied. According to the obtained results, at 15–15 MV energies, a significantly lower dose was absorbed by the thyroid gland, was observed in contrast to their counterparts who were treated with 6–15 MV photon beam energies.

Using 15–15 MV photon beam energies field can significantly reduce the absorbed dose to the thyroid gland and consequently can reduce the risk of developing hypothyroidism in breast cancer patients treated with radiotherapy.

The purpose of the study was to investigate the detailed angularly dependent attenuation characteristics of three different commercial couch-tops: Varian IGRT, Qfix kVue Standard and Qfix kVue Dose Max couch-tops used in radiation therapy.

The attenuation of photon beams by the treatment couch-tops was measured using a farmer chamber inserted at the centre of a 16 cm diameter cylindrical acrylic phantom for five different photon energies: 6 MV, 6FFF MV, 10 MV, 10FFF MV and 15 MV photon beams. The Varian IGRT couch-top has three different thicknesses thus attenuation measurements were done at the three different longitudinal locations. Measurements were made with the sliding support rails of the Qfix kVue Standard and Qfix kVue Dose Max couch-tops at both ‘rails-in’ and ‘rails-out’ positions. All measurements were taken for several projections through 360° movement of the gantry and for two different field sizes; 5×5 cm2 and 10×10 cm2.

The results indicate that the maximum attenuation of the Varian IGRT couch-top at the thin, medium and thick portions are 5·1, 5·7 and 8·9%, respectively, the Qfix kVue Standard couch with the rails-in and rails-out are 11·2 and 13·7%, respectively, and Qfix kVue Dose Max couch-top with rails-in and rails-out are 9·7 and 13·8%, respectively. The results from this study can be used to account for the couch-top attenuation during radiation treatment planning of patients treated with these couch-tops.

To evaluate the survival outcomes and toxicities experienced by non-metastatic head and neck cancer (HNC) patients receiving modulated radiotherapy (RT).

A total of 608 HNC patients treated consecutively from March 2010 to December 2014 with common subsites (oral cavity, oropharynx, hypopharynx, larynx and nasopharynx) of HNCs formed the study group. Eligible patients included those treated with radical or postoperative RT between March 2010 and December 2014. More than 90% patients received modulated RT [intensity-modulated radiotherapy (IMRT) or volumetric-modulated arc therapy (VMAT)] with concurrent chemotherapy as per stage guidelines. Demographic parameters and disease-related factors were analysed. Disease-free survival (DFS) was calculated from end date of RT till last follow-up or last date of disease control. Overall survival (OS) was calculated from date of registration to last follow-up date if alive. The primary endpoint was survival. The statistical analyses were performed using SPSS version 20.0 and Kaplan–Meier method was used for calculation survival.

Among the evaluable patients, the median age was 60 years (range: 16–93) with male preponderance (male:female – 513:95). Majority were squamous cell carcinoma 93·4% (568/608). The subsites treated were oral cavity 36·8% (224). oropharynx 26·4% (161), larynx 19·7% (120), hypopharynx 10% (62) and nasopharynx 6·4% (41). RT intent was radical in 63·5% (386) and postoperative in 36·5% (222), with 59·5% (362) receiving concurrent chemotherapy. At last follow-up, 348 (57·2%) patients were alive, 169 (27·7%) patients had succumbed to disease and 120 (24·6%) patients had recurrent disease. Out of 120 recurrent cases loco-regional recurrence, nodal recurrence and distant metastases were seen in 62 (51·7%), 25 (20·8%), 33 (27·5%), respectively. In the entire study cohort at 2 year OS and DFS was 80 and 79% whereas 3 years OS and DFS was 70 and 75%, respectively.

In our study, 2 years and 3 years OS and DFS rates are found comparable to the international data with acceptable toxicity profile with the use of modulated RT. It seems to be possible because of stringent departmental protocols and good medical physics support. Our data re-validates need and benefit of advanced RT techniques like IG-IMRT and VMAT for both postoperative and radical HNC treatment at the cost of minimal long-term side effects. Future stringent follow-up and quality of life issues are being considered in a prospective manner.

Dermatofibrosarcoma protuberans (DFSP) is a locally aggressive intermediate malignancy.

The purpose of this retrospective analysis is to determine the efficacy of radiation therapy (RT) in local control of DFSP.

The recurrence-free survival (RFS) for 45 patients treated for DFSP at our institution was estimated and compared between surgery alone and postoperative RT groups.

Age range of the patients were in the third and fourth decades; males:females=2:1; most common site: anterior abdominal wall; tumours >5 cm in size in 75%; low grade in 77·8%; margins positive in 31·8% and <5 mm margins in 45·5%. Two-thirds of patients had at least one recurrence before presentation to our institution. RT dose was >50 Gy in 88% of patients. The patients treated with postoperative RT had poorer prognostic factors compared with surgery alone: they were males (17 versus 13 patients), and presented with high-grade tumours (5 versus 1 patients), multiple recurrences prior to presentation (25 versus 20 patients) and positive or <5 mm margins (22 versus 12 patients). Median follow-up for surgery alone group was 17 (1–152) months and for postoperative RT group, this was 54 (5–121) months. RFS at 5 years was 77·1% for surgery alone and 87·9% for postoperative RT group but was not statistically significant. The median time to recurrence was 4 years.

RT delays the time to recurrence in DFSP. RT improves the outcome of DFSP for recurrent tumours and with positive margins.

The purpose of this study was to analyse the comparison of intensity-modulated radiation therapy quality assurance (IMRT QA) using Gafchromic® EBT3 film, Electronic portal imaging device (EPID) and MapCHECK®2.

Pretreatment authentication is the main apprehension in advanced radiation therapy treatment plans such as IMRT.

A total of 20 patients were planned on Eclipse treatment planning system using 6 and 15 MV separately.

Gamma index of EBT3 film results shows the following average passing rates: 97% for 6 MV and 96·6% for 15 MV using criteria of ±5% of 3 mm, ±3% of 3 mm and ±3% of 2 mm for brain. However, by using ±5% of 3 mm and ±3% of 3 mm criteria, the average passing rates were 95·4% on 6 MV and 95·2% on 15 MV for prostate. For EPID, the results show the average passing rates as 97·8% for 6 MV and 97·2% for 15 MV in for brain. In cases in which ±5% of 3 mm and ±3% of 3 mm were used, the average passing rates were 96·6% for 6 MVand 96·1% for 15 MV for prostate. MapCHECK®2 results show average passing rates of 96·4% for 6 and 96·2% for 15 MV, respectively, for brain using criteria of ±5% of 3 mm, ±3% of 3 mm and ±3% of 2 mm, whereas for ±5% of 3 mm and ±3% of 3 mm the average rates are 95·2% for 6 and 94·7% for 15 MV in prostate.

The EPID results are better than the other methods, and hence EPID can be used effectively for IMRT pretreatment verifications.

To determine the feasibility of an anthropomorphic breast polyurethane-based three-dimensional (3D) dosimeter with cavity to measure dose distributions and skin dose for a commercial strut-based applicator strut-adjusted volume implant (SAVI™) 6–1.

An anthropomorphic breast 3D dosimeter was created with a cavity to accommodate the SAVI™ strut-based device. 2 Gy was prescribed to the breast dosimeter having D95 to planning target volume evaluation (PTV_EVAL) while limiting 125% of the prescribed dose to the skin. Independent dose distribution verification was performed with GAFCHROMIC® EBT2 film. The dose distribution from the 3D dosimeter was compared to the distributions from commercial brachytherapy treatment planning system (TPS) and film. Point skin doses, line profiles and dose–volume histogram (DVHs) for the skin and PTV_EVAL were compared.

The maximum difference in skin dose for TPS and the 3D dosimeter was 4% whereas 41% between the TPS and EBT2 film. The maximum dose difference for line profiles between TPS, 3D dosimeter, and film was 4·1%. DVHs of skin and PTV_EVAL for TPS and 3D dosimeter differed by a maximum of 4% at 5 mm depth and skin differed by a maximum 1·5% between TPS and 3D dosimeter. The criterion for gamma analysis comparison was 92·5% at ±5%±3 mm criterion. The TPS demonstrated at least ±5% comparability in predicting dose to the skin, PTV_EVAL and normal breast tissue.

3D anthropomorphic polyurethane dosimeter with cavity gives comparable results to the TPS dose predictions and GAFCHROMIC® EBT2 film results in the context of HDR brachytherapy.

The data used in brachytherapy planning are obtained from homogeneous mediums. In practice, the heterogeneous tissues and materials affect the dose distribution of brachytherapy. It is aimed to investigate the effect of air cavities on brachytherapy dose distribution using a specially designed device.

In this study, the special device designed with different volumes of air and water to be irradiated and measured at different depths using EBT3 Gafchromic films. EBT3 Gafchromic films were preferred for this study because they can be cut to the shape of the experimental geometry, are water resistance and double directional usability.

In our study, sudden dose increases and decreases were observed at the water–air–water interfaces. Increases were 9, 11·8 and 15% in the 13, 18 and 22 mm apparatus, respectively. These effects were expected and the results were consistent with the literature and within the tolerance limits stated in the clinical dose guidelines. The most important result is that the percent depth–dose curve of the radiation passing through the air to the water and only passing through the water medium is different. The average differences were 1·97, 2·97 and 2·31% for the 13, 18 and 22 mm apparatus, respectively.

Although the effect of heterogeneity may be neglected according to clinical guidelines, it is suggested that the dose effect of heterogeneity is taken into account so that the dose can be estimated sensitively. Brachytherapy plans using dose data without considering air gaps may cause erroneous dose distributions due to heterogeneity of tissue.

To assess skin dose and incidence of skin reactions in early breast cancer patients treated via Intrabeam™ intraoperative radiation therapy (IORT) device.

In total, 250 breast cancer patients treated with a single fraction of 20 Gy using 50 kV photon were recruited. The applicator to skin distance (ASD) was measured before the initiation of the radiation and the skin dose in each patient was accordingly calculated based on the treatment planning system (TPS).

The average skin doses calculated were equal to 7·91, 5·83, 3·96 and 2·14 Gy for 6–10, 10–15, 15–20 and 20–30 mm ASD values, respectively. It is noticeable that the skin doses could be lower than the TPS measurements up to 45%, mostly due to lack of backscatter radiation in breast tissue compared with the full scatter condition in the Zeiss water phantom. Finally, only three patients showed low-grade skin reactions 1 week after IORT. A review of the related literature also revealed the incidence of lower skin complications among patients treated via Intrabeam™ compared with MammoSite™ machine.

The Intrabeam™ TPS did not seem to be very reliable for accurate skin dosimetry. However, breast cancer treatment using Intrabeam™ could result in fewer incidences of skin reactions than MammoSite™ machine.

To investigate the necessity of rotational shifts by considering dosimetric impact of rotational errors on stereotactic body radiation therapy (SBRT).

20 lung patients with the lesion size <5 cm treated with SBRT have been selected for dosimetric analysis. Three-dimensional dose has been rotationally shifted (±1°, ±3°, ±5° for pitch, roll and yaw) and overlaid to the original computed tomography images. The dose–volume histograms of 18-rotational plans of each patient were compared to those of the original plan.

No significant dosimetric differences were observed in target coverage. For all of the cases up to 5° in any couch angle dose differences of D99 and D95 were <3%. Variations of conformity index were observed to be less than 0·05. None of the organ at risk doses exceeded the dose limit. The V20 differences of the ipsilateral and the total lungs were less than 0·4%.

It has been found to be unnecessary to perform rotational shifts up to 5° for lung SBRT treatments; the translational shift is sufficient for the cases used in this study. This method may be applied and tested after planning and before treatment initiation to rule out exceptionally extreme cases.

The aim of this study was to understand how the regulatory requirements for functioning radiotherapy practices in India to control risk were conceptualised, perceived and applied accordingly in the radiotherapy facilities. It further examined how the social factors influenced the decision-making process for implementing regulatory requirements in the radiotherapy facilities.

This study was carried out in nine radiotherapy facilities located in the northeastern Indian states of Manipur, Assam, Meghalaya, Tripura and Mizoram. The study adopted both the semi-structured and in-depth questionnaire, developed on the basis of multidisciplinary fields.

The study found that the facilities in the northeastern regions were commissioned in line with the regulatory requirements. The facilities had adequate structural shielding rooms to protect workers, patients and the public from the risk of ionising radiation. However, in the operational phase of the facilities, majority of the facilities had the improper management of existing resources and non-implementation of regulatory requirements on time. It was observed that workers in some facilities continued the practice, despite the failure of specific safety functions, or not meeting regulatory requirements. Such practices led to the suspension of patient treatment in three of the facilities by the regulator. The existence of a varying nature of risk perceptions among oncologists, medical physicists, radiological safety officers, radiotherapy technologists in the facilities were observed and these influenced the decision-making process of the facilities on the implementation of regulatory requirements.

The study found that the facilities needed to explore various means, including to narrow the gap that existed in respects of perceived risk (within the facilities), communication to enhance work coordination and mutual trust among workers. The adoption of the institutional policy for conducting an internal audit of working practices, encouragement of workers to participate in continuing education programs would enhance effective utilisation of already existing infrastructure/equipment and work procedures including quality assurance programs.

In the United Kingdom, 7·1% of people aged over 65 live with dementia. The National Dementia Strategy emphasises the need for a skilled workforce competent in dementia care. In all, 50% of cancer patients are in the 70 plus age group, suggesting that education of therapeutic radiographers in the care of the patient with dementia is key. The aim of the study was to review the literature regarding training of therapeutic radiographers on dementia and use the findings to make recommendations for training in the future in order to provide the best care.

A literature search of electronic databases holding peer-reviewed journals was conducted. Search terms were generated using the Population Intervention Comparison Outcome (PICO) method and retrieved articles were evaluated using the Critical Appraisal Skills Programme (CASP) tool. Additional evidence was accessed through snowballing and from grey literature.

The search failed to find any studies on dementia care education within a radiotherapy setting and only a limited number were found in the acute care setting. There appeared to be a large variation in the education provision for healthcare professionals on the subject of dementia care, both at the pre-registration and post-registration stages.

There is no evidence of a standardised education programme for healthcare professionals in dementia care. In the future, therapeutic radiographers are likely to see more patients with dementia but currently some may not have had the education and training to provide effective holistic care.

Accurate three-dimensional dosimetry is essential in modern radiotherapy techniques such as volumetric-modulated arc therapy (VMAT) and intensity-modulated radiation therapy (IMRT). In this research work, the PRESAGE® dosimeter was used as quality assurance (QA) tool for VMAT planning for head and neck (H&N) cancer.

Computer tomography (CT) scans of an Image Radiation Oncology Core (IROC) H&N anthropomorphic phantom with both IROC standard insert and PRESAGE® insert were acquired separately. Both CT scans were imported into the Pinnacle (9.4 version) TPS for treatment planning, where the structures [planning target volume (PTV), organs at risk) and thermoluminescent detectors (TLDs) were manually contoured and used to optimise a VMAT plan. Treatment planning was done using VMAT (dual arc: 182°–178°, 178°–182°). Beam profile comparisons and gamma analysis were used to quantify agreement with film, PRESAGE® measurement and treatment planning system (TPS) calculated dose distribution.

The average ratio of TLD measured to calculated doses at the four PTV locations in the H&N phantom were between 0·95 to 0·99 for all three VMAT deliveries. Dose profiles were taken along the left–right, the anterior–posterior and superior–inferior axes, and good agreement was found between the PRESAGE® and Pinnacle profile. The mean value of gamma results for three VMAT deliveries in axial and sagittal planes were found to be 94·24 and 93·16% when compared with film and Pinnacle, respectively. The average values comparing the PRESAGE® results and dose values calculated on Pinnacle were observed to be 95·29 and 94·38% in the said planes, respectively, using a 5%/3 mm gamma criteria.

The PRESAGE® dose measurements and calculated dose of pinnacle show reasonable agreement in both axial and sagittal planes for complex dual arc VMAT treatment plans. In general, the PRESAGE® dosimeter is found to be a feasible QA tool of VMAT plan for H&N cancer treatment.

Intensity-modulated radiation therapy (IMRT) is one of the most reported techniques for head and neck cancer treatment, as it allows a good coverage of the planning target volume (PTV) while sparing the surrounding organs at risk (OAR) better than conventional conformal radiotherapy. The objective of this work is to optimise an IMRT technique for the simultaneously integrated boost (SIB) treatment of larynx cancer delivering a total dose of 69·96 Gy to the boost volume and 54·45 Gy to the elective volume in 33 fractions.

Three IMRT techniques, each using seven equally spaced beams, were planned for a sample of 10 patients. The first two techniques (IMRT-0 and IMRT-26) differ only for the starting angle of the seven beams, whereas the third (IMRT-CT) combines both these techniques by delivering IMRT-0 in the first half of treatment, and IMRT-26 in the second half, thus taking advantage of using 14 beams in total while using seven at a time only. The planning results were compared according to the dose coverage, homogeneity and conformity of the two PTVs, as well as to the dose to OARs, that is, spinal cord, parotids, mandible, brainstem and healthy tissue (defined as the body volume minus the sum of PTVs).

Basically the PTV coverage resulted acceptable and comparable with all the three techniques. Concerning OARs, statistically better results are obtained in IMRT-CT when compared with IMRT-26 and IMRT-0.

The IMRT-CT technique, combining two different seven-beam setups, delivered in two treatment phases, improves dose distribution without increasing delivery time.
