Using opioid for recreational purposes has a very long history in Iran. Social influence is a useful framework in understanding how the social environment affects the individual's behavioral choices to use drugs.

This study aimed to compare coping styles between opioid dependent and healthy controls.

This study compared opioid dependent individuals entering methadone maintenance treatment program (n=150) with healthy controls (n= 225) on measures of coping strategies with Jalowiec Coping Scale (JCS) and some socioeconomic factors. JCS consists of 40 coping behaviors, 15 problem focused and 25 affective focused items. Analysis of variances (ANOVA), multivariate linear regression and binary logistic regression were conducted.

Compared to controls, opioid dependent subjects reported less use of problem focused coping (P = 0.04). Control group were significantly more single than opioid dependent group (76.4% versus 23.6%; P = 0.01). There was a significant relation between opioid dependence with cigarette smoking (P < 0.001), lower education (P = 0.006), being employed (P < 0.001), having children (P < 0.001). Also, cigarette smokers had a significantly lower problem focused coping scale in comparing with non smokers in general (P < 0.001).

Opioid dependent subjects showed more vulnerable toward the stresses of life such as marriage, having children and a job. Educational programs to improve problem solving and coping skills can be helpful to reduce the rate of dependency to opioid and cigarette.

This year marks the 80th anniversary of the professional withdrawal (at the end of 1933) of Gustav Kolb (1870–1938). He is considered the founder of the ‘Erlangen System’ of open care.

Nearly 80 years after Gustav Kolb was forced to retire (1st March 1934), the poster pays tribute to Gustav Kolb's ‘Life's work’, by delineating the formation, active period and the fall of his ‘Erlangen System’ in its historical context.

Relevant archive materials and secondary literature were assessed.

Beginning in 1914, Gustav Kolb, as Director of the psychiatric facility in Erlangen (1911–1933) introduced care of the emotionally ill in theirown families. In 1930, 4,200 of the 770,000 residents in a catchment area covering about 3,200 square kilometers were being treated in open care. The ‘Erlangen System’ was the largest organization of its kind in Germany. Kolb opposed the national-socialist health politics, withdrew professionally in 1933 and died 5 years later.

Gustav Kolb's organizational thoroughness, with its creation of a central register of people under open care in the Erlangen System, provided considerable biogenetic information. Tragically, this was abused as an important source in carrying out the national-socialist law for prevention of genetically-impaired offspring (14.07.1933).

Individual efforts to reestablish open care facilities after 1945 were not implemented. It was not until during the sociopsychiatric movement of the 1960s that Kolb's concept could achieve a Renaissance, although it was unnamed and unrecognized at the time.

Le groupe de travail 3e cycle (GT3C) a été institué à la demande des ministères de la Santé et de l’Enseignement supérieur et de la Recherche et était composé de membres de la Commission nationale de l’internat et du post-internat et de la Commission pédagogique nationale des études de santé. Ses propositions ont été rendues publiques le 25 janvier 2014 [1]. Dans cette perspective, l’AFFEP, association représentative des internes de psychiatrie français, a conduit une enquête afin de mieux cerner les attentes des internes quant aux évolutions possibles de la maquette du diplôme d’études spécialisées (DES) de psychiatrie et du post-internat.

Un sondage a été soumis par courriel à tous les membres de l’AFFEP du 6 décembre 2013 au 12 février 2014.

Le taux de réponse était de 70 % (800 réponses) ; 30 % des internes étaient favorables à un allongement de la maquette à 5 ans et 42 % souhaitaient avoir deux stages en CHU obligatoires. Les internes souhaitaient la création de nouveaux diplômes d’études spécialisées complémentaires à 75 % en psychiatrie légale et 70 % en gérontopsychiatrie ; 15 % sont en faveur d’un DES indépendant de pédopsychiatrie.

Globalement, les internes de psychiatrie se montrent satisfaits de la maquette actuelle. Néanmoins, cette étude met en avant une nécessité de reconnaissance de sous-spécialité émergentes et un attachement au maintien de la pédopsychiatrie dans le DES de psychiatrie [2]. Cela correspond aux premières propositions du Collège national des universitaires de psychiatrie (CNUP) pour l’application de la réforme au DES de psychiatrie avec l’apparition des surspécialisations de pédopsychiatrie et de gérontopsychiatrie allongeant d’un an la durée du DES.

Les résultats de cette enquête ont permis de guider les réflexions communes de l’AFFEP avec le CNUP et le GT3C pour l’application de la réforme à l’internat de psychiatrie.

Manifestation of schizophrenia significant depends on whether the impact factors that trigger the disease on individual. The influence of protective factors can reduce the risk manifestation of schizophrenia and even improve clinical presentation.

To study the impact environmental factors, in risk and protective role, on manifestation and clinical presentation of schizophrenia spectrum disorders (F 20 - F 29).

We recruited 108 cases (50 females; 58 males) of schizophrenia spectrum disorders (F20 - F 29) aged 17 to 35 years. and control was included 46 risk group persons, who had genetic predisposition, but were not ill. The relatives and persons were interviewed for perinatal pathology and bird, child and adolescent history factors.

Among the main group environmental factors were predominantly: maternal age at birth of 40 years 20.37 % (N = 22), transferred diseases during mother pregnancy 11.1% (N = 12), the availability of social deprivation, child abuse and psychological trauma in early childhood 38.9% (N=42), consuming alcohol and cannabis in adolescence 19.4% (N=21). In addition, psychological trauma in childhood and using psychoactive substances are correlated with early social disadaptation in schizophrenia. On the other hand, the projective factors were high levels of intellect and social functioning 60.9% (N = 28), emotional support from parents 87% (n = 40), excluding the use of toxic substances 89.1% (N = 41).

Therefore, lifestyle changes that encourage minimized exposure to known environmental factors may reduce the risk for schizophrenia.

Dans le cadre de travaux sur la prise en charge de la dépression et des troubles anxieux, la CNAMTS s’est intéressée à l’activité des psychiatres libéraux.

À partir des bases médico-administratives nationales de l’assurance maladie (SNIIRAM), l’activité et la patientèle des psychiatres ont été analysées en 2013.

Quatorze mille trois cent psychiatres exerçaient en France en 2013, dont 8000 salariés et 6300 libéraux. Ces derniers étaient en majorité des hommes (60 %) à la moyenne d’âge élevée (57 ans), les 2/3 exerçant en secteur 1. Les disparités de répartition sur le territoire sont relativement importantes avec des densités élevées dans les grands centres urbains et dans le sud de la France. Ces déséquilibres ne sont pas sans conséquence sur l’activité des psychiatres. En 2013, les psychiatres libéraux ont assuré 14,5 millions de consultations pour 1,8 millions de patients (tous régimes, France entière). Parmi ces consultations libérales, on dénombrait un nombre important d’« honoraires de surveillance » réalisés en clinique (2,6 millions), mais peu de « consultations d’urgence » (12 300) réalisées à la demande du médecin généraliste par les psychiatres libéraux. La moitié des psychiatres libéraux ont réalisé 23 % de l’ensemble des consultations, alors que les 20 % les plus actifs en réalisaient 48 %. Peu de patients (4 %) ont nécessité plus de 40 consultations dans l’année, mais ces consultations fréquentes représentaient 27 % de l’activité des psychiatres libéraux. Les consultations ponctuelles (une ou deux consultations par patient) visant probablement à poser un diagnostic ou faire le bilan de prise en charge représentaient un tiers de la patientèle mais 4 % de l’activité.

La disponibilité en psychiatres libéraux est très variable sur le territoire. Un effectif modéré de psychiatres assure une part importante de l’activité libérale. Un effectif très modéré de patients bénéficie très fréquemment de cette activité libérale.

Borderline patients present complex clinical features, with neurotic and psychotic aspects so that literature often define them as ‘difficult’ or ‘at limit’ for pharmacological and psychotherapeutic treatments.

The aim of this study is to introduce the unconscious skills that emerge from dreams of early sessions as an integrative method of evaluation of the possibility for a psychodynamic psychotherapy treatment.

Three patients with previous psychiatric diagnosis of borderline personality disorder with psychotic traits and related drug therapies and/or support psychotherapies, began a psychodynamic psychotherapy with the goal of recovery and resolution of intrapsychic conflicts.

The unconscious reaction to proposition of recovery, instead of only support, was defined by dreams in which the functional, valid and affective structure of patients was prevalent, instead of disturbing and psychopathological aspects.

This unconscious awareness of patients, emerged as a reaction to a valid therapeutic alliance, allowed the continuation of the psychodynamic psychotherapy, that otherwise could be not efficacious or potentially risky as described in literature.

It is possible to start a psychodynamic psychotherapy for ‘difficult’ or ‘at limit’ patients, instead of usually supportive psychotherapies, for the unconscious reaction of a valid therapeutic alliance assessed through dreams of initial psychotherapeutic sessions.

So dreams regain a relevant role in diagnosis and prognosis for these patients, often ignored with decrease of treatment possibilities.

L’Organisation mondiale de la santé (OMS) a inscrit la participation de tous les acteurs concernés par la classification internationale des maladies (CIM) dans son processus de révision. L’implication des usagers et des aidants jusque-là partielle, est de plus en plus importante et implique une nouvelle méthodologie de révision où leurs rôles ne soient pas annexes mais essentiels, tout en respectant le cadre contraint de la révision. Des travaux préliminaires menés à Lille et à Montréal ont souligné l’importance de travailler avec les usagers et aidants sur les lignes directrices de la CIM et sur l’intégration d’éléments contextuels à la classification.

– Garantir la participation des usagers et aidants au processus de révision de la CIM.

– Intégrer des éléments contextuels à la classification.

– Proposer des recommandations à l’OMS permettant d’aboutir à une classification des troubles mentaux et du comportement compréhensible et acceptable par toutes les parties prenantes.

Étude internationale, formative et participative, multicentrique, conduite auprès des usagers, des aidants et des professionnels volontaires entre 2014 et 2015. Axes :

– intégration d’éléments contextuels dans la classification des troubles mentaux et du comportement;

– relecture de la classification sur le spectre de la schizophrénie et autres troubles psychotiques primaires.

Recueil des données : utilisation d’une triangulation de méthodes :

– réunions de travail et focus groupes;

– analyse de documents;

– questionnaire électronique.

Réalisation d’une analyse statistique, d’une analyse de contenu, et création de cartes mentales

Des recommandations claires et précises pour l’OMS seront élaborées à l’issue de ce projet. Celles-ci devront permettre d’aboutir à une classification des troubles mentaux et du comportement compréhensible et acceptable par toutes les parties prenantes, et sur le long-terme moins stigmatisante pour les usagers et aidants.

in psychodynamic group psychotherapy it is used to divide patients in different groups with direct access and/or after preliminary individual sessions. Since 2003 our research group started a new method. Aim of this study was to assess the value of this new methodology.

we use: psychodynamic approach, 10–14 patients, open access for pathology, age, sex, starting and ending. Therapy begins with individual sessions. Therapy deals with symptoms, intra-psychic conflicts, and private issues. Meanwhile psychiatrist develops relational capacity of the patient, allowing him to enter group therapy. We continue, at the same time, with individual and group therapy, for a time different for each case. Then therapy continues in the group only, until its ends. The duration of the three steps is variable, but the total amount of time of the therapeutic approach is reduced by half in 10 years.

Efficiency of this method is divided in three points. 1) Advantages for the single patient in the group: improvement of patient privacy; decrease of transferal resistance, acting-out and drop-out; economic advantages. 2) Advantages for group dynamics: decrease of aggressive behavior and interpersonal conflicts, improving of climate group; attenuation of hierarchal levels among patients, manipulation, fabrication, selfpity, narcissism. 3) Advantages for psychiatrist: continuous monitoring of the new patient or ‘difficult’ or risky patients with decrease of countertrasfert issues.

This methodology is an effective alternative to the usual group therapy. It allows a thorough and solid approach to the patient, offering a larger possibility of patient recovery.

A ‘Sleeping beauty’ is a scientific article that becomes highly cited long after its publication.

To present this phenomenon, not limited to biomedical research.

The most famous case of ‘Sleeping beauty’ was that of Gregor Mendel's seminal study on plant genetics that received widespread recognition 31 years after its publication.‘Sleeping beauties’ led to Nobel prizes (Herman Staudinger, Nobel in Chemistry 1953; Peyton Rous, Nobel in Chemistry 1966). They usually reflect premature discoveries that the scientific community was not ready to recognize when published. Some suppose that this has to do with most scientists’ tendency to adhere to their established paradigms. The article's authors may also be young and/or low in the hierarchy of science and their work is initially ignored. Perhaps the paper is not written in the right way for the right journal, il lacks clarity or is not adequately ‘promoted’ by its authors (e.g. in scientific meetings). Maybe the findings are difficult to be conceptually connected to the existing knowledge by a comprehensible and logical ‘bridge’. Sometimes a particular topic could be out of fashion only to see its popularity soar in future. ‘Sleeping beauties’ are thought to represent 100–1,000 articles out of nearly 1,000,000 papers published annually.

The public access granted to a lot of scientific articles makes difficult for a breakthrough paper to go unnoticed for long. However, just the quantity of today's publications could potentially ‘burry’ a great article.

This was a quasi experimental study which covered 34 Women with Multiple Sclerosis in Iran Multiple Sclerosis Society. The samples have been selected through purposive sampling and then Random assignment into intervention (16 Samples) and control group (18 Samples). Intervention was Yang style Tai Chi Chuan classes twice a week for 3 months (12 weeks). In this study demographic questionnaire, Berg Balance Scale (BBS) and Multiple Sclerosis quality of life questionnaire (MSQOL-54 scale) were used to collect data.

In the Tai Chi intervention group there was a significant improvement in mean comparison of balance score (P= 0.03) and global score of Quality of life (P = 0.00), also its subscales such as Pain (P = 0.03), Energy (P = 0.00), Overall quality of life (P = 0.01), Role limitation due to emotional problems (P = 0.04), Social Function (P= 0.03) and in Physical Health Composite Score (P = 0.03) after Tai Chi training intervention. (P< 0.05) In the control group, no statistically significant improvement was found in mean comparison of balance and Quality of life scores after 12 weeks. (P > 0.05)

The results suggested that Tai Chi Chuan could be an alternative exercise intervention to increase balance and quality of life in patient with Multiple Sclerosis.

Au cours d’un épisode dépressif, un arrêt maladie peut permettre de gérer les situations de stress. Cependant, la prolongation excessive de l’arrêt peut aggraver l’anxiété et entraîner une désocialisation, mais aussi témoigner de la gravité de la maladie. Notre objectif est de décrire les patients avec arrêt de longue durée pour dépression, analyser leur recours aux soins, identifier des facteurs prédictifs des arrêts, et suivre leur devenir.

Les bases de l’assurance maladie (SNIIRAM/PMSI) ont été utilisées pour constituer une cohorte de bénéficiaires du régime général, âgés de 20–69 ans, débutant en 2011 un arrêt maladie > 6 mois pour dépression et sans antécédent d’arrêt long en 2010 pour ce motif.

En 2011, 84 000 personnes (2,6 pour 1000) étaient en arrêt maladie long pour dépression. Parmi celles-ci, 34 000 (1,1 pour 1000) l’avaient débuté en 2011 et constituent la cohorte (âge moyen 46 ans, femmes 66 %). L’arrêt était prescrit dans 75 % des cas par un médecin généraliste, 10 % un psychiatre libéral et 12 % un médecin salarié. Dans l’année précédant l’arrêt, 27 % avaient consulté un psychiatre libéral, et 57 % dans l’année précédant ou suivant le début de l’arrêt. L’année précédente, 43 % des patients avaient eu ≥ 3 délivrances d’antidépresseur et 83 % l’année suivante. Pour 39 % on retrouvait : ≥ 1 comorbidité somatique identifiée par une ALD ou une hospitalisation (23 %), une ALD psychiatrique (21 % dont 89 % pour dépression), une hospitalisation en MCO à ± 3 jours de l’arrêt (5,5 % dont 6 % pour tentative de suicide et 14 % pour pathologie psychiatrique).

Un recours au psychiatre libéral est observé dans les 2 ans (avant/après) pour environ la moitié des 34 000 patients ayant débuté un arrêt maladie de longue durée pour dépression en 2011. Les analyses sont en cours pour déterminer quels facteurs influencent le recours à ces arrêts et quelle en est l’évolution (invalidité éventuelle).

Negative symptoms, a core feature of chronic schizophrenia, are notoriously resistant to treatment. New therapeutic strategies are proposed based on the hypothesis of a relation between glytamatergic neurotransmission and schizophrenia.

Case report.

Male patient, 51 years old, suffering from schizophrenia since the age of 22. He was stabilized on risperidone long-acting injection -37.5mg/2 weeks- and biperiden 4mg/day (due to persistent extrapyramidal tremor). His prominent symptoms were the negative ones: Affective flattening, avolition, asociality and poverty of speech. Memantine 10mg/day was added. One and a half month later the patient spontaneously admitted to spend more time with his relatives enjoying their company more than in the past. He was assessed with the Scale for the Assessment of Negative Symptoms (=96), the Scale for the Assessment of Positive Symptoms (=3), the Mini Mental State Examination (=26) and the Calgary Depression for Schizophrenia Scale (=2). Memantine was increased to 20mg/day. Two months later, affective flattening and asociality continued to improve; avolition and poverty of speech remained generally unchanged (SANS: 76, SAPS: 1, MMSE: 26, CDSS: 1). No additional side-effects were noted.

Memantine blocks the excessive influx of calcium through the activated N-methyl-D-Aspartate (NMDA) receptors which are part of the glutamatergic system. Neuroprotection of the glutamate neurons could hypothetically ameliorate several symptoms of schizophrenia that are thought to be mediated by them. In the literature the findings regarding its addition to atypical antipsychotics are conflicting, however it is proposed as potentially helpful for patients with severe residual psychopathology.

Our study was descriptive and analytic, involving 63 patients with schizophrenia.

We used a questionnaire containing demographic, clinical, therapeutic, and habits data. Smoking have been evaluated by the FTND (Fagerstrom test of nicotine dependence); with a total score ≥ 7 indicating a strong dependence.

The average age was 43 years (21- 63); the sex ratio (H/F) was 3, 09. They were singles in 52.4%. The predominant form of schizophrenia was paranoid in 65, 1% of case. The patients were smoking in 50,8% ;with an average number of cigarettes :14,62/day (0-70), the age of smoking initiation was 9.80 years. They have a family smoking history in 63,5% of cases. An attempts to weaning were revealed in 41,3% of our patients. There was an alcohol consumption in 28.9% of cases and cannabis use in 11.1% of cases. The results of FTND test : not dependents (4,8%), weakly dependents (6,3%), moderatly dependents (14,3%) et strongly or very strongly dependents (23,8%).the factors were correlated with smoking: the sex (p=0,00), the age of smoking initiation (p=0,02), the smoking motivation (p=0,00)the number of cigarettes/day (p=0,00), the alcohol consumption (p=0,00), Side effects of treatment (p=0,03) and the equivalent dose of chlorpromazine (p=0,002). ;

In Patients with schizophrenia, addictive behaviors are common especially smoking. The treatment of this psychiatric comorbidity, factor increased dependence is essential to this. There are many advanced management including substitution programs, withdrawal and CCT.

Le recours au psychiatre libéral est peu décrit en termes de nombre de consultations et de caractéristiques de la population.

Les patients du régime général d’assurance maladie ayant consulté au moins une fois un psychiatre libéral en 2012 ont été identifiés dans les bases médico-administratives nationales, afin de décrire les diagnostics d’ALD ou d’hospitalisation, consommations de soins et caractéristiques démographiques.

Près de 1 433 400 personnes (2,4 %) ont consulté au moins une fois un des 6290 psychiatres libéraux en 2012. L’âge médian était de 44 ans avec une majorité féminine (64 %). Le nombre moyen de consultations par patient dans l’année était de 10; 20 % (n = 286 300) avaient consulté 1 fois, 49 % (n = 699 800) 2–9 fois, 19 % (n = 266 500) plus de 15 fois. La proportion des bénéficiaires de la CMU-c de 18–59 ans était de 8 % contre 12 % en population générale. Un quart des patients avaient un diagnostic de maladie psychiatrique identifiable par leur ALD ou une hospitalisation (18 % par l’ALD23 seule). Un diagnostic de troubles psychotiques était retrouvé pour 5 % des patients, dépression pour 10 % et troubles maniaques et bipolaires pour 4 %. Près de la moitié (46 %) avaient eu ≥ 3 remboursements d’antidépresseur ou lithium dans l’année et 16 % ≥ 3 antipsychotiques, et 35 % ni diagnostic identifiable ni ces traitements. La fréquence des comorbidités somatiques différait peu de celle de la population générale.

Deux à trois personnes sur 100 consultent un psychiatre libéral dans l’année. Pour un quart seulement un diagnostic peut être identifié par une ALD ou une hospitalisation, mais des traitements antidépresseur (46 %) ou antipsychotique (16 %) sont fréquemment retrouvés. Près de 20 % des patients n’ont consulté qu’une seule fois en 2012 probablement pour avis, et sensiblement autant ont consulté plus de 15 fois.
