Dissociative experiences have been studied in different circumstances.

In this study we aim to analyze dissociative phenomena under a stress factor: lockdown.

We conducted a cross sectional-study, using an online survey, spread during lockdown period in Tunisia, between the 2nd and the 8th of April. It was comprised of sociodemographic, geographic, medical history, confinement status and DES-II questionnaire in its french version. Age superior than 18 was the only inclusion criteria and the no respect of lockdown was the exclusion criteria. Based on former studies on DES-II, 3 sub scores have been assessed; amnesia (measures memory loss), depersonalization (sense of unreality of the self) and absorption (the absorption has to do with one’s traumatic experiences).

We recruited 167 individuals; 100 women and 67 men. The most common age class was 20 to 30 years old (60.5% of the sample). The Mean total score was 11.06 which was higher than mentioned in earlier studies. The mean score was 15,11 for absorption; 5,28 for amnesia and 6,88 for depersonalization subscale. Significant differences in scores were found based on different variables. Women had higher absorption score (p=0.011). Besides people living in COVID-19 clusters had lower total score (p=0.038). Finally, people with somatic medical history showed higher total score (p=0.013), absorption score (p=0.003) and depersonalization score (p=0.012) compared to those with none.

During lockdown, dissociative experiences showed to be more frequent. But does this mean that a resurgence in PTSD (posttraumatic stress disorder) or ASD (acute stress disorder) in the months to come.

Clinical case of 10-year-old patient with anorexia nervosa at the stage of severe cachexia. Features of the disease, diagnosis, treatment and methods of restoration of nutrition. Anorexia nervosa is an eating disorder (ED), severe pathology, manifested by severe complications, high disability and can cause death. ED has traditionally been perceived as a disease affecting women. However, this pathology occurs men, is 10-25% of the total number of patients with ED, or 1-2% in the population.

The clinical case indicates the need to increase the attention of pediatricians, psychologists, psychiatrists and other doctors in connection with the growth and rejuvenation of ED in the male population.

Some attempts are being made to describe the mental health and well-being of the population in relation with the regional specifics in a particular area of the Russia, however, they are rare and local (limited to one region).

The current study aims to identify sociocultural mediation of mental well-being based on its measures in Russia’s regions with different ethno-cultural composition of population.

The study involved 210 men and 403 women aged 14 to 76 years (M = 26.9; SD = 13.7) from six regions of the Russian Federation: Moscow, St. Petersburg, Udmurtia, Sakha, Sverdlovsk and Kemerovo. The mental well-being of participants was assessed using the Warwick-Edinburgh Mental Wellbeing Scale (Tennant et al., 2006; Tennant et al., 2007).

The measures of mental well-being were different in the regions: Moscow (M=51.04; SD=8.03), St.-Petersburg (M=50.05; SD=5.78), Udmurtia (M=47.57; SD=9.50), Sakha (M=50.99; SD=8.47), Sverdlovsk (M=54.86; SD=8.43), Kemerovo (M=51.84; SD=7.51). Using one-way analysis of variance (ANOVA), it was found that there are significant differences in the assessment of psychological well-being between study participants from different regions (F = 6.692; p <0.001). Residents from the Sverdlovsk evaluate their mental well-being as significantly better than the ones from Moscow (MD=3.821; p=0.012), St.Petersburg (MD=4.812; p=0.023), the Udmurtia (MD=7.284; p <0.001) and the Sakha (MD=3.869; p=0.003).

Residents from Russia’s regions with different ethno-cultural composition of population demonstrate some difference in mental well-being measures that may be caused by sociocultural factors. The reported study was funded by the Russian Foundation for Basic Research, project number 17-29-02506.

Benzodiazepines (BZD) are widely used in patients with bipolar disorder (BD) and their effectiveness is well documented. Therefore, there are major risks associated with BZD use including abuse and dependence. Those risks can be related to the patients’chacteristics, the particularities of BD and the prescribers.

To determine the factors associated with chronic use of BZD in patients with BD.

We conducted a cross-sectional, descriptive and analytical study among a sample of patients with BD (DSM-5) followed in psychiatric outpatient of Hedi Chaker university hospital in Sfax. We used the Benzodiazepine Cognitive Attachment Scale (ECAB) to determine dependent patients

Among the 61 included patients, 50 (82%) had a chronic use of BZD (> 3 months). They had a mean age of 49.3 years (± 14.02 years) and a low socio-economic level in 44%. The type of BD was dominated by type II (66%). Initial episode type was depressive in 78%. The average number of depressive episodes was 2.92±2.3. A rate of 65.5% of patients have already attempted BZD withdrawal. Chronic BZD use was significantly correlated with BZD dependence (p=0.000), low socioeconomic level (p=0.04), depressive type of the initial episode (p=0.011), the depressive recurrence (p=0.000) and the absence of any attempt to discontinue BZD (p=0.011).

Chronic use of BZD in patients with BD is prevalent. In order to minimize this problem in this population, it is important to enhance programs to improve psychiatrist-prescribing behavior and to use cognitive-behavioral therapies in combination with medication to help withdrawal.

The psychological stress associated with the COVID-19 pandemic has a complex multifactorial nature.

The aim: to evaluate the level of stress during the COVID-19 pandemic in the Russian-speaking population with(-out) affective disorders in different cities of residence.

The data obtained from an online survey of 4803 Russian-speaking respondents (age over 18) from March 30 to May 18, 2020. The survey included social, demographic and the history of affective disorders data. The anxiety distress level was evaluated with the Psychological Stress Measure (PSM-25).

Individuals from sub-cohort of Russian cities with populations less than one million had higher stress levels (M=135.39) compared to Moscow (M=129.47; p=0.003) or St.-Petersburg (M=126.63; p<0.001). However, stress scores in respondents with a history of affective disorders correspond to the average stress level according to PSM-25. Respondents without affective disorders from St.-Petersburg reported lower stress levels (M=92.88) than respondents from Moscow (M=100.47; p<0.001) and Russian cities with less than one million population (M=98.4; p<0.001). Average stress scores from St.-Petersburg and other Russian cities show a low level of stress on PSM-25, which indicates psychological adaptation. Stress scores from Moscow have borderline values between low and medium levels.

Our study showed that the city of residence and affective disorders status significantly affect stress levels in Russian population. These factors could be further used in individual psychological support strategies.

She will then describe the program, which consists of a “hub and spokes” model with “tele-clinics” consisting of a “hub” of specialists and “spokes” of clinicians in rural, underserved areas who present cases for discussion, generating treatment recommendations.

The ECHO model has been replicated in a variety of disciplines across the United States and internationally. Its success has been well documented. There are currently 920 active ECHO programs worldwide.

Project ECHO is a viable model to address the workforce shortage of child psychiatrists worldwide.

18 patients of cluster 1 (C1) demonstrated explicit self-stigmatization. In comparison with 25 subjects from cluster 3 (C3) stigmatized patients (C1) had higher levels of overall ISMI scores (2.9±0.3) caused by alienation (3.1±0.5), stereotype endorsement (2.5±0.5), social withdrawal (2.7±0.4), and discrimination experience (2.7±0.4). 20 patients of cluster 2 (C2) had an implicit stigma. They were more self-stigmatized (ISMI score 2.7±0.3) in contrast with subjects from cluster 3 (1.9±0.2) due to a lower level of stigma resistance (C2: 3.8±0.5 and C3 3.1±0.6 – reverse scores). Patients with implicit self-stigma (C2) had the lowest intensity of treatment motivation (Z-scores -1.2±0.6) compering with others (C1 and C3) due to the lowest TMAQ factor 1 (reliance on own knowledge and skills to cope with the disorder: -1.0±0.6) and factor 4 (willingness to cooperate with doctor: -0.9±1.0). Differences between explicitly and implicitly stigmatized patients manifested also in lower TMAQ factor 3 for the second group (awareness of the psychological mechanism of maladaptation: -0.5±0.9).

Despite alienation, stereotype endorsement, social withdrawal, discrimination experience some patients could sustain stigma due to cooperation with doctors and reliance on their own knowledge and skills to cope with illness.

The Childbirth Expectations Questionnaire (CEQ; Gupton, A., Beaton, J., Sloan, J. & Bramadat, I.; 1991) evaluates the women childbirth expectation’s with 34 items organized in four dimensions: Pain and coping; Significant others; Nursing support and Interventions.

To analyze the psychometric properties (construct validity using Confirmatory Factor Analysis, discriminant validity and reliability) of the Brazilian preliminary version of CEQ.

350 women (Mean age: 30.01±5.452) in the second trimester of pregnancy (Mean weeks of gestation=25.17±6.55), with uncomplicated pregnancies, completed the CEQ. To analyze discriminant validity, thirty of these women participated in a workshop (12 hours, integrated in the GentleBirth, a specific perinatal education intervention program) and fill in the CEQ again after approximately 8 weeks.

After deleting seven items (1-3-20-24-33-34-35) and some errors were correlated the four-dimensional second-order model of CEQ presented good fit (χ2=2.496; RMSEA=.071; CFI=.845, TLI=.828). The CEQ Cronbach’s alpha for the total was α=.90; all factors presented good reliability: Pain coping (α=.87); Significant others (α=.66), Nursing support (α=.84), and Interventions (α=.76). The CEQ mean scores (total, Pain coping and Nursing support) were significantly higher after the workshop, indicating more positive expectations for childbirth (p<.05).

This additional validation study emphasizes that CEQ is an adequate measure of expectations of labour. It will be very useful to understand the correlates of childbirth expectations and also to access the efficacy of childbirth preparation programs.

In a serial compound conditioning paradigm, a sequence of several conditioned stimuli (CS) is predictive to an unconditioned stimulus (US) (e.g., CSA->CSB->US). Animal research showed that, when the US is aversive, CSA elicits the strongest conditioned response, while CSB appears redundant. These effects of primacy and proximity have never been investigated in humans.

To study the effects of temporal proximity of imminent threat and safety in serial compound conditioning.

Twenty-two participants were presented with sequences [CSA->CSB->CSC->CSD]. In 55 trials all four CS were identical vowels (e.g, [oh]), and no US was presented. In the other 55 trials, the CSA was different (CSA+, e.g., [uh]), and the CSD was followed by an electrical shock (US) 2.5 times higher than the individual pain threshold.

No ERP component distinguished between CS- and CS+ for the first three stimuli in the sequence (i.e., CSA, CSB, CSC). The last CS (CSD) elicited a strong fronto-central CNV only when it was followed by US. Moreover, already after the CSA- (which signalized that no shock would be presented on that trial) the power of alpha oscillations over the somatosensory cortex significantly increased, particularly on the side contralateral to the hand that was electrically stimulated on US trials. The alpha increment lasted up to the onset of the US.

The data indicate two possible mechanisms of adjustment to predictable threat, one of which relies on safety signals (manifested in alpha increment), and the other is related to flight response (manifested in the CNV immediately preceding the shock).

In response to the spread of COVID-19, many Brazilian therapists faced the challenge of taking their practices online considering legal and ethical issues, besides learning to handle new technologies in a way the therapeutic setting was maintained. The cooperation of the family is fundamental for the creation and maintenance of an adequate therapeutic setting. Children are not sufficiently mature to speak clearly about what bothers them or to talk about how they feel and why, so, drawing, pretend playing, story telling, playing games are the common tools for children’s communication during therapy.

Evaluating if online therapy for children can support therapeutic play tools and be effective in a virtual environment preserving the therapeutic setting.

Two children aged 6 to 11 attended the psychological sessions that were conducted through video calls.The family should provide a silent and private room for those sessions. The children were free to choose the toy they would like to play with and that was available at home such as board games, comic and story books. Mimicry, drawing, an adaptation of the Winnicott Squiggle Game were used, as well as electronic games through screen sharing.

The emotional conflicts were expressed either through conventional games and play or electronic games. Playing with children online was possible as well as maintaining the therapeutic alliance in order to carry on with the treatment in a proper therapeutic setting.

Online therapy for kids showed to be an effective form of service delivery, under strict measures of social distancing in Brazil.

Due to COVID-19 pandemic, the Tunisian government officially announced a lockdown on March 2020. This decision caused a significant change in the everyday life of Tunisians such as movement restriction revealing the psychosocial aspect of this crisis.

This study aims to assess the impact of the COVID-19 pandemic on the mental health of Tunisian women and its association with social media addiction.

This study was conducted using an online survey, between April 25 and May 6, 2020. Women were asked about sociodemographic information, lockdown conditions. The Facebook Bergen Addiction Scale (FBAS) was used to evaluate addiction to Facebook and social media. The Depression Anxiety and Stress Scales (DASS-21) was used to evaluate depression, anxiety and stress.

We included 751 participants. Scores of the FBAS ranged from 6 to 30 and the mean score was 16.49 (± 5.4). Forty percent (n = 300) of respondents might have facebook addiction, as per the scale. A significant positive correlation was found between Facebook addiction score and DASS scores of depression (p = 0.001, r = 0.43), anxiety (p = 0.001, r = 0.39) and stress (p = 0.001, r = 0.41).

Women who have higher rates of distress are more likely to have a problematic Facebook use during the COVID-19 pandemic which suggests that smartphone and internet use may be part of coping strategies implemented for the emotional distress secondary to this pandemic.

The only FDA approval therapeutic for schizoaffective disorder is paliperidone. Hiperprolactinemia is one of the most frequent side effects induced by first generation antipsychotics (FGA) or by second generation antipsychotic (SGA), such as risperidone and paliperidone. Prolactin related symptoms (PRS) include amenorrhea, galactorrhea, gynecomastia and fluctuations in psychotic symptoms.

To report the case of a patient with schizoaffective disorder difficult to manage due to symptom resistance and PRS, that improved symptomatology when prolactin serum levels were reduced.

Clinical-demographic data collected by clinical interview and clinical process consultation. Non-systematic literature review, searching “psychosis”; “prolactin”; “antipsychotic”; “schizoaffective disorder” on Pubmed database.

We report the case of 33 years-old female, admitted to our psychiatry inpatient unit for persecutory delusions, loosening of association, auditory hallucinations, and irritability with functional impairment. Symptoms began 13 years before. She was medicated with paliperidone 100mg IM monthly, lithium 800mg daily and clozapine 225mg daily. When admitted she wasn’t adhering to oral medication. On physical examination presented some PRS. The serum presented hyperprolactinemia and lithium in non-therapeutic levels. Initially was re-introduced the previous therapeutic without improval. It was made a therapeutic switch to associate aripiprazole 400mg IM monthly and clozapine 225mg daily, and lithium 800mg daily resulting in prolactine normalization and subsequent improval of psychotic symptoms previously presented.

This case reports challenges in management of patients diagnosed with Schizoaffetive Disorder due to therapeutic refractoriness and side effects. PRS can be ruling, therefore impacting therapeutic choices. We propose a possible role of combination of clozapine and aripiprazole in this scenario.

Adults with ADHD describe self-medicating with cannabis. A small number of psychiatrists in the US prescribe cannabis medication for ADHD, despite there being no evidence from trials. The EMA-C trial (Experimental Medicine in ADHD-Cannabinoids) was a pilot randomised placebo-controlled experimental study of a cannabinoid medication, Sativex Oromucosal Spray, in 30 adults with ADHD. The primary outcome was cognitive performance and activity level using the QbTest. Secondary outcomes included ADHD and emotional lability (EL) symptoms. From 17.07.14-18.06.15, 30 participants were randomly assigned to the active (n=15) or placebo (n=15) group. For the primary outcome, no significant difference was found in the intent-to-treat analysis although the overall pattern of scores was such that the active group usually had scores that were better than the placebo group (Est=-0.17,95%CI-0.40-0.07, p=0.16, n=15/11 active/placebo). For secondary outcomes Sativex was associated with a nominally significant improvement in hyperactivity/impulsivity (p=0.03) and a cognitive measure of inhibition (p=0.05), and a trend towards improvement for inattention (p=0.10) and EL (p=0.11). Per-protocol effects were higher. Results did not meet significance following adjustment for multiple testing. One serious (muscular seizures/spasms) and three mild adverse events occurred in the active group and one serious (cardiovascular problems) adverse event in the placebo group. Adults with ADHD may represent a subgroup of individuals who experience a reduction of symptoms and no cognitive impairments following cannabinoid use. While not definitive, this study provides preliminary evidence supporting the self-medication theory of cannabis use in ADHD and the need for further studies of the endocannabinoid system in ADHD.

A signifiant proportion of adolescents and young adults suffer from mental disorders that interfere with their development and influence their academic and professional succes.

To describe the soscio-demographic and clinical profile of tunisian students who have consulted in the psycho-pedagogy unit.

A retrospective descriptive study, data were collected from the files of 359 pupils and students having consulted in the psycho-pedagogy unit of the psychiatry (B) department of the Hedi Chaker university hospital of Sfax in Tunisia, from 2014 to 2018.

The average age of students was 20.38 ±2.38 years old. The Sex ratio = 0.95 (175 M / 184 W). Students were undergraduates in 55.4 of the cases. THey are smokers in 19% and consume alcohol in 7% of case. A family psychiatric history was present in 34% of students and personal history in 25%. A difficult childhood was found in77.5% of patients and their academic results were low (< 10/20) in 54%. Adjustement disorders were the most frequent disorders in psycho-educationel counseling (47% of case). The most prescribed psychotropic drugs were antidepressants (31.8%) followed by anxiolytics (23.7%) and the withdrawel syndrome was found in 18% of patients.

A students with a difficult childhood, low academic results and adjustement disorders such is the profile most frequently encountered among patients consulting the psychopedagogy unit. In this way,the promotion and protection of adolescent’s health particulary pupils and students benefits not only for their own health, but also for the economic and the society.

Attachment styles play important role in managing health behavior. It has been observed in researches that attachment style have relationship with health behavior (Schlack, 2003). If attachment styles are left the way they form habitually it can be harmful, in a study with diabetic patients, people with avoidant attachment style were expected to die within 5 years of disease diagnosis (Ciechanowski et al., 2010).

To assess effect of adult attachment styles on health behavior?

Sample comprised of 300 university students from different private and government universities of Karachi with age range 18 – 35 years. Assessment tools used are relationship questionnaire and wellness behavior inventory scale. Relationship questionnaire is used to identify the dimension of attachment style (Bartholomew & Horowitz, 1991). Wellness behavior inventory was used to identify consistency of healthy behavior activities done on regularly basis (Sirois, 2001). Statistical tests used for descriptive analysis were frequency and percentage and for inferential statistics regression analysis test was used.

According to the attachment styles A, B, C and D most of the study participants fell in healthy weight range, a few were in obese range which is considered unhealthy. Result of regression analysis estimated there is no effect of attachment style on health behavior as p-value was greater than 0.05.

Attachment style is not a good predictor of health behavior solely. As per a few researches in order to study impact of attachment styles other mediating variables that can have effect on health behavior should also be observed such as self-esteem.

Therapeutic effect of Qinghuanling on negative symptoms and cognitive function of schizophrenia

To evaluate the therapeutic effect of Qinghuanling on cognitive impairment in schizophrenia, and to provide basis for clinical medication.

24 male patients with schizophrenia were randomly divided into study group and control group. The study group was given quetiapine fumarate combined with Qinghuanling, and the control group was given quetiapine fumarate. The positive and negative symptom scale (PANSS) and adverse event response scale (TESS) were evaluated regularly.

The PANSS score of the study group was significantly lower than the control group from 6th week (64.10 ± 7.64 vs 72.31 ± 11.16; 51.60 ± 7.40 vs 63.23 ± 7.08, P < 0.05). Among them, the score of negative factor in the study group was significantly lower than that in the control group at the end of 6 and 8 weeks (2.16 ± 0.40 vs 2.75 ± 0.38; 1.65 ± 0.42 vs 2.38 ± 0.43, P < 0.01); the score of cognitive factor in the study group was significantly lower than that in the control group at the end of the 8th week (1.87 ± 0.20 vs 2.12 ± 0.27, P < 0.05). Compared with before treatment, PANSS score and symptom cluster factor score of the two groups were significantly decreased from the 2nd weekend to the 8th weekend (P < 0.05).

The combined use of Qinghuanling can significantly improve the therapeutic effect of schizophrenia, especially for the symptom cluster score of negative factors and cognitive factors, with high safety.

Tension-type headaches is the most common type of headache among adults and it rises a challenge in findig an effective and safe treatment method.

The study aims to evaluate the corporal acupuncture therapy efficacy in patients with chronic tension-type headaches undergoing a complex treatment plan.

The study involved 132 patients (74% female and 26% men) aged 18-65 years, who were divided into two groups. Patients reported their pain lasted 0.4-12 years. All the patients received conventional treatment (central muscle relaxants and antidepressants). The study group additionally received classical corporal acupuncture 3 times per week, a course of 12 sessions. Treatment effectiveness was evaluated by measuring pain intensity using a subjective visual analogue scale (VAS) and McGill Pain Questionnaire (MPQ).

Most of the patients (79% and 88% of study and control groups respectively) demonstrated moderate cervical musculoskeletal dysfunctions. At admission pain intensity was 4.2±1.5 and 3.8±1.7 VAS points in the control and study groups respectively, MPQ sensory rank pain index (RPI) was 5.92±1.49 points, affective RPI 3.41±0.84, the total RPI – 7.12±2.56 in the control group, and 6.22±1.74; 2.98±0.62 and 7.14±1.65 points in the study group. 4 weeks after treatment measurements showed following pain intensity changes: 4.21±0.74 vs 3.1±0.95 points in the control and study groups respectively. 3- and 6-month period revealed 3.1±0.57, 2.4±0.74 points and 2.1±0.62, 1.1±0.49 points in the control and study groups respectively.

Classical corporal acupuncture course may benefit chronic tension-type headaches patients providing an effective treatment in a safe way.

Delusion of pregnancy has been described as a false and fixed belief of being pregnant despite factual evidence to the contrary. Pseudocyesis is a condition in which the patient has all signs and symptoms of pregnancy except for the confirmation of the presence of a fetus. There has been described symptoms as abdominal distention, cessation of menses, morning sickness, etc.

The aim of this work was to distinguish between pseudocyesis and pseudopregnancy. This case presents a single 49-year-old woman, who developed the delusion of being pregnant after months of lonely confinement during Covid-19 quarantine.

She had missed her period for 10 months. She was convinced that she could feel the fetal movements. Her thought content revealed delusion of persecution, reference, and delusion of being pregnant. She did not reveal any hallucinations. Blood tests and brain imaging revealed no abnormalities. □The treatment was started with Paliperidone 100mg/month, and clonazepam 2mg/d.

The patient showed a substantial improvement within 10 weeks of treatment. Pseudocyesis and delusional pregnancy have been rarely described on scientific bibliography. A distinction has been demonstrated based on the consideration of the associated psychotic features that might be present in delusions of pregnancy, which were described in the current case. On the other hand, pseudocyesis clinical presentation is centered on the false signs and symptoms of pregnancy.

Antipsychotics played a key role in the delusion of pregnancy. Psychodynamic and supportive psychotherapy could play a pivotal role in the management of pseudocyesis.

Review of effective interventions for improving emotional risk factors In infertile and infertile patients undergoing treatment

A comprehensive narrative review of the studies was conducted. Databases such as Web of Science, Science Direct, Cochrane Library, Scopus, PubMed, including Medline, Clinical key, SID, MAGIRAN were retrieved from August 10 to December 8, 2020, with no time limit. After reviewing the abstract and the full text of the articles in terms of compliance with the purpose of the study, finally, 32 articles were selected for writing. The methodological quality of the articles was assessed based on Cochrane Risk of Bias

Interventions were divided into two subgroups of mind-body, and web-based CBT. mind-body interventions generally shows the anxiety,stress and depression reduction and Possible improvement in pregnancy rate But most of these programs require extensive financial resources The results of web based, showed that using online CBT approach can greatly reduce stress and anxiety, due to increased use of internet, non collaborative,cheap and private treatment of web based interventions, this method can be used as a way along with other treatments to reduce these negative reactions

According to the present study CBT methods, application and Internet-based interventions can be used as appropriate counseling methods in reducing stress, anxiety and improving pregnancy outcomes in infertile patients. This information can be used as a proper source to select appropriate counseling methods for health care providers, midwives and treatment staff involved in infertility patients
