Historical survey data suggest that the seroprevalence of antibodies against Coxiella burnetii in the general population of The Netherlands decreased from more than 40% in 1983 to 2·4% in 2007, just before the start of the large 2007–2010 Q fever epidemic. To assess whether the sharp decline in seroprevalence was real, we performed a cross-sectional study using historical samples. We tested samples using a contemporary commercial indirect immunofluorescence assay. In plasma samples from the south of The Netherlands from 1987, we found an age- and sex-standardized seroprevalence of 14·4% (95% confidence interval 11·2–18·3). This was significantly lower than a 1983 estimate from the same area (62·5%), but significantly higher than 2008 (1·0%) and 2010 (2·3%) estimates from the same area. The study suggests that there was a steady and sharp decline in Q fever seroprevalence in the south of The Netherlands from 1987 to 2008. We assume that seroprevalence has decreased in other parts of The Netherlands as well and seroprevalence surveys in other European countries have shown a similar declining trend. Waning population immunity in The Netherlands may have contributed to the scale of the 2007–2010 Q fever epidemic. For a better understanding of the infection dynamics of Q fever, we advocate an international comparative study of the seroprevalence of C. burnetii.

Transmissible gastroenteritis (TGE) is a viral disease causing dehydration, diarrhoea and death in pigs. The disease is widespread in pig-producing areas of the world but does not occur in Australia. A mathematical model of TGE spread within a pig herd is proposed and calibrated by reference to published data. The model is then applied to two situations of special interest; first to estimate the delay before detection of TGE (6 to over 30 days) when infection is first introduced into a herd of domestic or feral pigs, and second the effect of the disease in a population of feral pigs (could become endemic if transmission is high).

Human rotavirus strains from Kenya, from children with gastroenteritis in an urban area (Nairobi) and three rural areas were characterized by antigenic and genomic analysis. While in all areas strains with subgroups II and G serotype 1 antigens were most common, two unusual strains were detected. One strain (NK59: subgroup II. G serotype 4) possessed an additional RNA band on polyacrylamide gel electrophoresis, the other (D202) which had antigenic specificity of subgroup II and G serotype 1 showed a ‘short’ RNA pattern. The latter strain was adapted to growth in cell culture.

1. Earlier attempts to trace the vector of tropical typhus in Kenya failed. The only references to the subject in the available literature consist of mere suggestions that a mite would most likely prove to be the transmitter.

2. An investigation made in an area whence most Nairobi cases of tropical typhus were reported, suggested that a tick (R. pulchellus) would be the most likely vector.

3. Transmission experiments made in the belief that one of the unclassed fevers of man was conveyed by R. pulchellus have so far yielded negative results. There is, however, sufficient circumstantial evidence available pointing to this tick as vector of a form of mild typhus to man—this demands further investigation.

4. At Mombasa and Nairobi, houses reported to be heavily infested with ticks, or houses investigated after the occurrence of the tropical typhus in them, have yielded only R. sanguineus.

5. R. sanguineus (3 ♀), taken from a dog in a house where the last typhus case had occurred 8 months previously, gave a typical typhus syndrome when emulsified and inoculated into a male guinea-pig. R. sanguineus (1 ♀, 12 ⊙), taken in a house where a child had recently contracted typhus, also gave a positive result with guinea-pigs and the virus was further transmitted by passage through other guinea-pigs.

6. The infestation of houses by R. sanguineus and the incidence of tropical typhus among human beings appear to be influenced by unfavourable weather conditions, causing the ticks to seek relatively dry and warm places for purposes of oviposition or metamorphosis, thus invading houses. In the absence of dogs, its usual hosts, the tick attacks man.

Floor dust has been collected for bacteriological examination by three methods: (1) with a miniature vacuum cleaner in which the ‘bag’ consists of a paper Soxhlet extraction thimable, (2)by sweeping, and (3)by blowing the dust on to the surface of a culture plate. The dust on to the surface of a culture plate. the dust from on to the first two methods was shaken with broth and an aliquot sample plated out on crystal-violet blood ager plates.

Surface inoculation of the plates was found to give somewhat higher counts than the use of pour plates, but otherwise minor variations in the routine were without great effect on the count.

Analysis of the results obtained in examining a number of specimerns from schoolrooms and day nurseries showed that, though the counts of streptococci in successive samples from one dust suspension agreed well, there was wide variation between the counts from duplicate samples of dust, however collected; the frequency distribution of the counts in rooms was of the log-normal type. It is clear that in routine work, all the dust collected from the whole floor of a room should be examined, in order to give the most reliable result.

Of 185 specimens in which four colonies of haemolyt streptococci were grouped, 67% had all four strains of group A. Ofniety-four samples in which four group A strains were typed, 55 % had all four strains of the same type; more than one type was found in 20 %.

Neisser in 1906 claimed to have discovered an organism which, when grown on lactose-agar, threw off suddenly a new strain capable of fermenting lactose. The identity of the new strain was established by Massini (1907) by many cultural and serological tests, and from that time onwards it has been generally accepted that new bacterial strains of great permanence may arise so suddenly as to justify this type of variation being named “ Mutation.”

An outbreak of salmonellosis, involving cases of infection with Salmonella enteritidis phage types (PT) 5c and 6a, occurred across Scotland between May and August 2000. In total, 70 outbreak cases were microbiologically confirmed. Preliminary investigation suggested that consumption of food, especially chicken dishes, from Chinese restaurants or take-aways (food businesses) was a risk factor for infection. A matched case-control study demonstrated a statistically significant association (OR 22·4, P=0·0024) between infection and consumption of food from Chinese food businesses. A cohort study of novel design suggested that chicken was an important vehicle of infection. However the result did not reach statistical significance (OR 1·7, P=0·3). Extensive environmental investigation was unable to identify the source of the suspected contaminated chicken.

The high incidence of norovirus (NoV) infections seems to be related to the emergence of new variants that evolved by genetic drift of the capsid gene. In this work, that represents a first effort to describe the molecular epidemiology of NoV in the northwest of Spain, a total of eight different NoV genotypes (GII.1, GII.3, GII.4, GII.6, GII.7, GII.12, GII.13, GII.14) were detected. The major genotypes observed were GII.4 (45·42%) and GII.14 (34·9%), being detected in all age groups. In addition, and although most of GII.4 sequences belonged to 2006b (7·2%) and 2010 (50·35%) variants, the presence of new NoV variants was observed. Phylogenetic analysis revealed that a high number of GII.4 sequences (35·24%) could be assigned to the newly emerging Sydney 2012 variant, even during late 2010. The high prevalence of NoV GII.14 observed in this study may indicate the emergence of this genotype in Spain.

Various explanations have been proposed to account for the increased incidence of moniliasis as a result of ‘broad spectrum’ antibiotic therapy, but it is generally believed that the suppression of the bacterial flora enables fungi to proliferate without competition. The pathogenicity of Candida albicans is, in any event, associated with the numbers of cells present and we have undertaken tests in vitro in an attempt to contribute more evidence on the mechanism of their proliferation in the presence of tetracycline-treated bacterial flora. The results are presented here as a preliminary communication.

Leptospirosis is endemic in the Andaman Islands, often occurring as outbreaks during the post-monsoon period. Pulmonary involvement is common and associated with high morbidity and mortality. During the investigation of an outbreak in North Andaman in 1996 an isolate was recovered from the blood of a patient with fever, headache, body aches and haemoptysis with respiratory distress as presenting symptoms. The isolate was characterized using the cross-agglutination absorption test (CAAT) and monoclonal antibodies (mAbs). The isolate showed typical morphology and characteristic motility of the genus Leptospira. Growth was inhibited at 13 °C and in the presence of 8-azaguanine. The isolate could not be identified with grouping sera representing 25 serogroups, CAAT and mAbs. A new serovar of a new serogroup is proposed. Genetic characterization using polymerase chain reaction (PCR) followed by sequencing of the PCR product and randomly amplified polymorphic DNA fingerprinting (RAPD) showed that the isolate was genetically similar to L. interrogans sensu stricto.

Primary care clinicians have a central role in managing influenza/influenza-like illness (ILI) during influenza pandemics. This study identifies risk factors for influenza-related complications in children presenting with influenza/ILI in primary care. We conducted a cohort study using routinely collected linked data from the Clinical Practice Research Datalink on children aged 17 years and younger who presented with influenza/ILI during the 2009/10 pandemic. We calculated odds ratios (ORs) for potential risk factors in relation to influenza-related complications, complications requiring intervention, pneumonia, all-cause hospitalisation and hospitalisation due to influenza-related complications within 30 days of presentation. Analyses were adjusted for potential confounders including age, vaccination and socio-economic deprivation. Asthma was a risk factor for influenza-related complications (adjusted OR 1.48, 95% confidence interval (CI) 1.21–1.80, P < 0.001), complications requiring intervention (adjusted OR 1.44, 95% CI 1.11–1.88; P = 0.007), pneumonia (adjusted OR 1.64, 95% CI 1.07–2.51, P = 0.024) and hospitalisation due to influenza-related complications (adjusted OR 2.46, 95% CI 1.09–5.56, P = 0.031). Neurological conditions were risk factors for all-cause hospitalisation (adjusted OR 4.25, 95% CI 1.50–12.07, P = 0.007) but not influenza-related complications (adjusted OR 1.46, 95% CI 0.83–2.56, P = 0.189). Community-based early interventions to prevent influenza-related clinical deterioration should therefore be primarily targeted at children with asthma and neurological conditions.

Previous evidence has suggested an association between cryptosporidiosis and consumption of unfiltered drinking water from Loch Katrine in Scotland. Before September 2007, the water was only micro-strained and chlorinated; however, since that time, coagulation and rapid gravity filtration have been installed. In order to determine risk factors associated with cryptosporidiosis, including drinking water, we analysed data on microbiologically confirmed cases of cryptosporidiosis from 2004 to 2010. We identified an association between the incidence of cryptosporidiosis and unfiltered Loch Katrine drinking water supplied to the home (odds ratio 1·86, 95% confidence interval 1·11–3·11, P = 0·019). However, while filtration appears to be associated with initially reduced rates of cryptosporidiosis, evidence suggests it may paradoxically make those consumers more susceptible to other transmission routes in the long-term. These findings support implementation of similar treatment for other unfiltered drinking-water supplies, as a means of reducing cryptosporidiosis associated with drinking water.

1. Bacteriological examination of large samples of Perch and Powan from natural and artificial waters has shown that a considerable decline in the normal bacterial flora of the fish occurs during the summer months. Reinfection occurs when the temperature falls.

2. These variations are largely attributable to the effect of temperature upon the immunological responses of the fish.

3. The bacteria concerned appear to be water bacteria, which invade the muscles and body cavity of the fish, as well as the gill and gut-content.

4. The degree of infection is also influenced by the bacterial content of the water.

The coronavirus disease 2019 (COVID-19) pandemic is currently the most critical challenge in public health. An understanding of the factors that affect severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection will help fight the COVID-19 pandemic. This study sought to investigate the association between SARS-CoV-2 infection and blood type distribution. The big data provided by the World Health Organization (WHO) and Johns Hopkins University were used to assess the dynamics of the COVID-19 epidemic. The infection data in the early phase of the pandemic from six countries in each of six geographic zones divided according to the WHO were used, representing approximately 5.4 billion people around the globe. We calculated the infection growth factor, doubling times of infection and death cases, reproductive number and infection and death cases in relation to the blood type distribution. The growth factor of infection and death cases significantly and positively correlated with the proportion of the population with blood type A and negatively correlated with the proportion of the population with blood type B. Compared with the lower blood type A population (<30%), the higher blood type A population (⩾30%) showed more infection and death cases, higher growth factors and shorter case doubling times for infections and deaths and thus higher epidemic dynamics. Thus, an association exists between SARS-CoV-2 and the ABO blood group distribution, which might be useful for fighting the COVID-19 pandemic.

We conducted a cross-sectional seroepidemiological study in 2012–2013 to determine the seroprevalence of varicella-zoster virus (VZV) in adolescents and adults living in Korea, where varicella vaccination has been recommended universally at age 12–15 months since 2005. Residual serum samples were collected from 1196 healthy adults and adolescents aged ⩾10 years between November 2012 and March 2013. The fluorescent antibody to membrane antigen (FAMA) test and enzyme-linked immunosorbent assay (ELISA) were performed to determine the seroprevalence of VZV. The seroprevalences of VZV were compared between six age groups: 10–19, 20–29, 30–39, 40–49, 50–59, and ⩾60 years. The seroprevalence of VZV in the entire study cohort was 99·1% according to the FAMA test and 93·1% as determined by ELISA. The seroprevalences of the six age groups were as follows: 96·0%, 99·5%, 99·5%, 99·5%, 100%, and 100%, respectively, by the FAMA test, and 83·3%, 93·0%, 93·0%, 97·5%, 94·5%, and 97·5%, respectively, by ELISA. Seroprevalence increased significantly with age (P < 0·001); moreover, the seroprevalence in subjects aged 10–19 years was significantly lower than in other age groups (P < 0·001), as measured by both the FAMA test and ELISA. Thus, strategies to increase protective immunity against VZV in teenagers are necessary.

Salmonella is one of the most prevalent bacteria associated with enteric illness in Canada and seniors are considered a vulnerable population more likely to develop severe illness. In the coming decades, hospitalizations and deaths associated with Salmonella in seniors could represent a challenge due to an aging population in Canada. The numbers of non-typhoidal (NT) Salmonella-related hospitalizations from the Canadian Hospitalization Morbidity Database were analysed for a period of 10 years for seniors. Hospitalization rate calculations and descriptive analyses were performed on variables associated with the burden of hospitalization and compared with the adult age group. Estimates of hospitalizations and deaths associated with domestically acquired Salmonella (accounting for under-reporting) were also calculated. This study found that 50% of the NT Salmonella-related hospitalization and 82% of the deaths recorded in the Canadian adult population occurred in seniors. The length of hospitalization stay was also longer in seniors (7 days) than other adults (4 days). We estimated that each year, 535 hospitalizations and 27 deaths are related to domestically acquired Salmonella in seniors. Senior populations represent a substantial percentage of Salmonella-related hospitalizations and deaths in Canada and the burden associated with those hospitalizations is also greater. This should be considered when developing estimates of medical costs and implementing prevention activities.

The epidemiology of West Nile virus (WNV) in Israel is different from other neighbouring countries in the Middle East where disease burden has been minimal. We analysed a cohort of Ashkenazi Jewish patients with symptomatic WNV infection (n = 39), and WNV-negative controls (n = 61), for nine genetic variants that has been suggested to be associated with susceptibility to WNV. Two single nucleotide polymorphisms were significantly more frequent in WNV-infected than non-infected individuals, rs7280422 (MX1) [odds ratio (OR) 4·05, 95% confidence interval (CI) 2·04–8·03, P < 0·001] and rs3213545 (OASL) (OR 1·85, 95% CI 1·03–3·3, P = 0·03). Genetic polymorphism may play a significant role in susceptibility to WNV infection in Ashkenazi Jews

The Australian brushtail possum is the major source of infection for new cases of bovine tuberculosis in cattle in New Zealand. Using hypothetical values for the cost of putative cattle and possum Tb vaccines, the relative efforts required to eradicate Tb in cattle using possum culling, possum vaccination or cattle vaccination are compared. For realistic assumed costs for 1080 poison bait, possum culling is found to be a cost-effective strategy compared to cattle vaccination if the required control area is below 13 ha per cattle herd, while possum vaccination is cost-effective for control areas of less than 3 ha per herd. Examination of other considerations such as the possible roles of possum migration and heterogeneities in possum population density suggest that each control strategy may be superior under different field conditions. Finally, the roles of the possum in New Zealand, and the Eurasian badger in Great Britain and Ireland in the transmission of bovine tuberculosis to cattle are compared.

Enterovirus A-71 (EV-A71) may be fatal, but the natural history, symptoms, and signs are poorly understood. This study aimed to examine the natural history of fatal EV-A71 infection and to identify the symptoms and signs of early warning of deterioration. This was a clinical observational study of fatal cases of EV-A71 infection treated at five Chinese hospitals between 1 January 2010 and 31 December 2012. We recorded and analysed 91 manifestations of EV-A71 infection in order to identify early prognosis indicators. There were 54 fatal cases. Median age was 21.5 months (Q1−Q3: 12–36). The median duration from onset to death was 78.5 h (range, 6 to 432). The multilayer perceptron analysis showed that ataxia respiratory, ultrahyperpyrexia, excessive tachycardia, refractory shock, absent pharyngeal reflex, irregular respiratory rhythm, hyperventilation, deep coma, pulmonary oedema and/or haemorrhage, excessive hypertension, tachycardia, somnolence, CRT extension, fatigue or sleepiness and age were associated with death. Autopsy findings (n = 2) showed neuronal necrosis, softening, perivascular cuffing, colloid and neuronophagia phenomenon in the brainstem. The fatal cases of enterovirus A71 had neurologic involvement, even at the early stage. Direct virus invasion through the neural pathway and subsequent brainstem damage might explain the rapid progression to death.

In order to establish the differences in transmission pattern of varicella-zoster virus (VZV), a comparative seroepidemiological study was carried out in two different children samples. Children aged 1–11 years, were randomly selected from state schools of São Paulo city, Brazil. Individuals aged 1–15 years were sampled by cluster from Caieiras city. Children aged 3 years or under from Caieiras were not attending school, while those from São Paulo were attending all-day nurseries or kindergarten. The presence of antibodies to VZV was analysed by ELISA technique. The force of infection and contact rate were determined by mathematical techniques. The average age of first infection was 2·87±0·14 years and 4·07±0·47 years for São Paulo and Caieiras, respectively. The average force of infection estimated was 0·29 year−1 for São Paulo and was 0·26 year−1 for Caieiras. The proportion of seropositivity and the force of infection were higher in São Paulo school children up to 3 years of age compared with Caieiras children, where the social contact starts later. In conclusion, social changes affecting contact among children may influence varicella epidemiology.
