Mental health disorders are common in pregnancy and after childbirth with over 10% of women manifesting some form of mental illness during this time. Maternity services will encounter women with symptoms that vary in severity from mild self-limiting to potentially life-threatening. These conditions carry risks for both the woman and the fetus/newborn. Detecting women with, or at risk of, a serious mental health disorder and enabling them to access appropriate care in a timely fashion is a shared responsibility. However, given the frequency of contact they have with women through this period, maternity services have a pivotal role. From a mental health perspective, high-risk pregnancies are those primarily associated with serious mental illness (psychotic illnesses, bipolar disorder and severe depressive episodes). Healthcare professionals caring for pregnant women should have the appropriate skills to detect serious mental illness and identify women at risk and how to access specialist mental health care.
Place has been central to sociolinguistic research from the beginning. How speakers conceptualize and orient to place can influence linguistic productions. Additionally, places can and do have myriad meanings – some strongly contested. Further, place is not static, as people move and the ideologies regarding certain places evolve over time. This Element probes these themes. It begins by reviewing the existing work on language and place within sociolinguistics according to key themes in the literature – place orientation, gentrification, globalization, and commodification, amongst others. Then it introduces key concepts and frameworks for studying place within allied fields such as geography, sociology, architecture, and psychology. Each author then presents a case study of language and place within their respective field sites: rural Appalachia and Greater New Orleans. Mudie's Select Library was a major nineteenth-century literary institution. Substantially larger than its competitors, the library leveraged regional and global distribution networks and close commercial ties with publishers which allowed it to maintain a key position within the British publishing industry. In its heyday, it was widely believed that novelists and publishers were required to conform to aesthetic, moral and formal standards established by Mudie's, or risk the rejection and consequent failure of their books. However, the lack of a comprehensive study of the library's holdings leaves open questions about what the library actually stocked, and to what extent the library could determine a novel's fate. This Element describes a data analysis of a collection of Mudie's catalogues spanning eighty years, in order to reassess understandings of the library's role in the nineteenth-century publishing industry. Multispecies Justice (MSJ) is a theory and practice seeking to correct the defects making dominant theories of justice incapable of responding to current and emerging planetary disruptions and extinctions. Multispecies Justice starts with the assumption that justice is not limited to humans but includes all Earth others, and the relationships that enable their functioning and flourishing. This Element describes and imagines a set of institutions, across all scales and in different spheres, that respect, revere, and care for the relationships that make life on Earth possible and allow all natural entities, humans included, to flourish. It draws attention to the prefigurative work happening within societies otherwise dominated by institutions characterised by Multispecies Injustice, demonstrating historical and ongoing practices of MSJ in different contexts. Good communication is necessary for good clinical care, but defining good communication has been surprisingly difficult and controversial. Many current ideas that identify good communication with certain communication behaviours, or 'skills', were ethically inspired to help doctors see beyond disease to the whole patient. However, promoting specific behaviours is problematic because communication is contextually dependent. In recent decades, observational research into practitioner-patient relationships has begun to provide a scientific basis for the field, identifying patients' vulnerability and practitioners' authority as defining features of fundamentally asymmetric clinical relationships. Future educators can learn from research that explores the judgments that experienced practitioners make when they manage communication dilemmas arising from this asymmetry. In future, instead of the current emphasis on teaching communication behaviours, educators could provide practitioners with knowledge about relationships to inform those judgments, while addressing the attitudes and values that motivate and guide their communication.
The British women booksellers who built and ran successful businesses before, during, and after the Second World War have largely been forgotten. This Element seeks to reclaim some of these histories from where they lie hidden or obscured in archives, accounts of the book trade of the time, and other sources. Though they were often called 'formidable', this research reveals astonishing impact at local, national, and international levels. Divided into four main sections, the Element first gives a literature review of materials about booksellers, before giving a short context to bookselling, the book trade, and book buyers and readers of the early twentieth century. A third section examines the position of women in society at that time, including how they were viewed as part of the book trade; the final section provides histories of nine women booksellers. This study explores the relationship between crime fiction and adoption. A primary goal of this study is to investigate how the adoption trope reveals current cultural fears and fascination with kinship formations and essentialist notions of belonging, DNA and ancestry searches, and the controversial practice of adoption. Popular fiction, namely crime/detective fiction and the subgenre of adoption crime fiction, reflect scholarly debates within the field of Critical Adoption Studies, including but not limited to the ethics behind the adoption industrial complex and calls for its demise, struggles for reproductive justice, and redefinitions of parenting and kinship formations evidenced by LGBTQ and racialized identities. Another goal of this study is to highlight adoptee and birthparent voices and perspectives, illustrating how genre fiction can help to agitate for more inclusive representation. By intersecting elements of crime into the adoption story, these narratives illuminate the human cost and condition of current adoption practices.
Misinformation can be broadly defined as information that is inaccurate or false according to the best available evidence, or information whose validity cannot be verified. It is created and spread with or without clear intent to cause harm. There is well-documented evidence that misinformation persists despite fact-checking and the presentation of corrective information, often traveling faster and deeper than facts in the online environment. Drawing on the frameworks of social judgment theory, cognitive dissonance theory, and motivated information processing, the authors conceptualize corrective information as a generic type of counter-attitudinal message and misinformation as attitude-congruent messages. They then examine the persistence of misinformation through the lens of biased responses to attitude-inconsistent versus -consistent information. Psychological inoculation is proposed as a strategy to mitigate misinformation.
This chapter covers generalized anxiety disorder, panic disorder, specific phobia, social anxiety disorder, agoraphobia, separation anxiety disorder, and selective mutism. Based on a review using the new criteria for empirically supported treatments, there is strong research support for: (a) exposure therapies for specific phobias, (b) cognitive and behavioral therapies for generalized anxiety disorder, (c) cognitive-behavioral therapy for panic disorder, and (d) cognitive-behavioral therapy for social anxiety disorder. The two primary components of treatment include exposure and addressing cognitive biases. Each of these components is broken down into additional parts. A sidebar also discusses acceptance and commitment therapy.
Obsessive-compulsive disorder (OCD) is a neurobehavioral condition that can lead to functional impairment and decreased quality of life. In this chapter, clinical presentation, diagnostic considerations, and pathophysiology of OCD are reviewed. An overview of the theoretical models of OCD are provided, and evidence-based treatments for OCD, specifically cognitive behavioral therapy (CBT) with exposure and response prevention (ERP), pharmacotherapy, and neurosurgery, are discussed. The chapter concludes with suggestions for future research directions.
Chapter 7 acknowledges that, despite the best planning for positive engagement, students will still exhibit disengaged and disruptive behaviours. It examines the research to discuss which behaviours are the most common and the most difficult to manage in a classroom environment. It makes the distinction between frequent disengaged behaviour and rare ‘challenging’ behaviour discussed in Chapters 9 and 10.
Building on previous chapters, this chapter also discusses the best ways to prevent disengaged behaviours through implementing consistent classroom routines, structures and expectations, including the explicit teaching of expected behaviour. Ongoing strategies such as social-emotional learning to build strong relationships, low-key techniques to remind and redirect behaviours, class meetings to support student voice and engaging lessons are explored.
In September 2008, the oldest investment bank on Wall Street, Lehman Brothers, declared bankruptcy. Immediately, the world’s financial system seized up. Hundreds of billions of dollars’ worth of financial assets were frozen in place, the value of securities made uncertain, and the solvency of seemingly rock-solid financial institutions called into question. By the end of 2008, the United States’ economy was in freefall, shrinking at an annualized rate of 8%. Growth rates in other major industrialized economies also plummeted as well. The recession was so deep, and the recovery so labored that it took more than a decade for output to return to full employment levels. Figure 19.1 illustrates the situation rather dramatically.
In the early neoclassical era, the smooth adjustment of agents’ marginal utilities dampened the response of market prices to supply disturbances. But when agents do not make smooth trade-offs, they will tend consume goods in specific combinations, as in the model of safety bias. A negative shock to supplies can then send prices soaring, a common feature of energy markets for example. Postwar general equilibrium theory permits agents to be safety-biased but has dismissed these volatility scenarios on the grounds that they occur only at vanishingly unlikely output levels. The no-volatility conclusion fails to hold however once the production of goods is taken into account: the seemingly unlikely output levels arise systematically. Production on the other hand spells out countervailing forces that curb the most extreme cases of price volatility. Volatility is closely related to the indeterminacy of equilibria and the chapter critiques the claims of general equilibrium theorists that these phenomena are unlikely.
